体位干预在玻璃体切割手术治疗糖尿病视网膜病变中的应用研究

Application of body position intervention in vitrectomy for diabetic retinopathy

目的:探讨体位干预在玻璃体切割手术治疗糖尿病视网膜病变(DR)患者中的应用效果。方法:将2014年1月~2017年1月80例择期玻璃体切割术的DR患者随机等分为观察组和对照组。对照组给予常规体位干预;观察组由手术室护士对患者进行体位干预,术后病房护士配合干预。观察两组术后正确俯卧位持续时间、舒适度及视网膜复位率。结果:观察组术后24,72 h及1周俯卧位持续时间均长于对照组,且术后不适度轻于对照组(P<0.05);观察组因俯卧位引起的颈肩部酸痛、烦躁、肢体麻木等发生率低于对照组(P<0.05);两组术后视网膜复位率比较差异无统计学意义(P>0.05)。结论:手术室护士参与行玻璃体切割手术治疗DR患者的术后体位干预,有利于提高术后俯卧位依从性及舒适度。

Objective：To explore the effect of body position intervention in vitrectomy for diabetic retinopathy（DR） patients.Methods：A total of 80 patients with DR undergoing elective vitrectomy from January 2014 to January 2017 were randomly and equally divided into observation group and control group.The control group was treated with routine body position intervention;the observation group was treated with body position intervention by theatre nurses,with combined intervention from the the ward nurses.The duration of the prone position,the degree of comfort and the retinal reattachment rate were observed in the two groups.Results：The duration of the prone position 24 h,72 h and 1 week after operation in the observation group was longer than that in the control group,and the postoperative discomfort was lighter than that in the control group（P〈0.05）;the incidence of soreness of neck and shoulder caused by the prone position,irritability and numbness in the observation group was lower than that in the control group（P〈0.05）;the difference of the retinal reattachment rate in the two groups was not statistically significant（P〉0.05）.Conclusion：The participation of theatre nurses in vitrectomy for DR patients after body position intervention is conducive to improving compliance and comfort degree of postoperative prone position.
