欣母沛联合卡孕栓预防巨大儿产妇剖宫产术产后出血的临床分析

Clinical Analysis of Hemabate Combined with Carboprost in Preventing Postpartum Hemorrhage of Macrosomia in Cesarean Section

目的分析卡前列素氨丁三醇(欣母沛)联合卡前列甲酯栓(卡孕栓)防治巨大儿产妇剖宫产术中及术后出血的效果及安全性。方法方便选取2015年2月—2016年8月呼和浩特市第一医院40例巨大儿产妇剖宫产术胎儿,娩出后按使用宫缩剂不同分为两组,对照组和试验组,各20例,两组均宫体注射缩宫素20 IU,对照组同时宫体注射欣母沛250μg;试验组同时宫体注射欣母沛250μg+舌下含服卡孕栓1 mg。观察两组术中、术后2 h的出血量及总出血量并定量,并观察不良反应。结果两组手术时间差异无统计学意义(P>0.05),两组术中出血量、术后2 h出血量及总出血量均有显著性差异(P<0.05),试验组术中、术后2 h、总出血量分别为(286.5±67.3)mL,(87.6±32.7)mL,(374.0±97.5)mL,而对照组分别为(347.8±99.6)mL,(115.4±48.1)mL,(463.2±119.3)mL。不良反应对照组6例,试验组5例,主要为恶心、呕吐,腹泻,血压轻度升高,短时间内消失。结论在常规使用缩宫素基础上,剖宫产术中联合使用欣母沛和卡孕栓,术中、术后出血量减少,不良反应少。

Object ive This paper tries to observe the effect and safety of hemabate combined with carbopros t in the treat -ment of cesarean sect ion and postoperative bleeding of macrosomia. Methods 40 cases of maternal cesarean sect ion from February 2015 to August 2016 in Hohhot First Hospital were convenient selected and randomly divided into two groups, the control group and the experimental group, with 20 cases in each group, the control group was given the uterus inject ion of oxytocin 20 IU combined with hemabate 250 滋 g; th e ex p erim en ta l group was tre a ted with h em ab a te of 2 5 0 滋 g and th e su b -l ingual carpros t of 1 mg. The blood loss and total blood loss dur ing and two hours af ter the cesarean sect ion were calculated and the adverse reactions were observed. Results There was no s ignif icant difference in the operat ion time between the two groups （P〉0.05）. The total blood loss dur ing and two hours af ter the cesarean sect ion and the total blood loss of the experimental group were （286.5 ±67.3）、 （87.6 ± 3 2 .7 ） 、（374 ±97.5）mL, while th e control group were （347.8 ±99.6）mL, （115.4±48.1）mL, （463.2±119.3）mL. As for the adverse, 6 cases in the control group, and 5 cases in the exper imental group, mainly accompanied with nausea, vomiting, diar rhea, mildly elevated blood pressure, disappeared spontaneous ly within a short period of time. Conclusion In the convent ional use of oxytocin, the combination of hemabate and carbopros t in cesarepos tope ra tive b leeding , with fewer ad v erse re actions.an section can re d u c e th e amount of in trao p e ra tiv e and pos tope ra tive b leeding , with fewer ad v erse re actions.
