锁定钢板治疗胫骨远端Pilon骨折的手术疗效

Curative Effect of Locking Plate in Treatment of Distal Tibial Pilon Fracture

目的探讨对胫骨远端Pilon骨折患者行锁定钢板治疗的临床效果。方法方便选取2015年12月—2016年12月该院收治的88例胫骨远端Pilon骨折患者作为研究对象,根据治疗方法的不同分为治疗组和常规组,各44例。常规组行常规钢板治疗,治疗组行锁定钢板治疗,观察并比较两组治疗效果、各项指标的变化情况以及并发症的发生几率。结果治疗组的总体有效率42例(95.45%)显著高于常规组34例(77.27%),治疗组的手术时间(46.4±2.9)min、术中出血量(40.8±4.7)mL、踝关节功能评分(82.3±4.5)分、骨折愈合时间(17.8±0.4)周、正位片成角(1.5±0.6)°以及侧位片成角(1.2±0.4)°等各项指标均优于常规组(52.4±5.3)min、(76.2±5.4)mL、(72.6±3.8)分、(26.4±1.6)周、(4.2±0.9)°以及(3.2±0.7)°,治疗组发生并发症几率3例(6.82%)显著低于常规组11例(25.00%),差异有统计学意义(P<0.05)。结论对胫骨远端Pilon骨折患者行锁定钢板治疗,不仅能够提高患者的治疗效果,同时,还能够在一定程度上减少并发症的发生,利于患者病情的快速康复。

Objective To study the clinical effect of locking plate in treatment of distal tibial Pilon fracture. Methods 88 cases of patients with distal tibial Pilon fracture admitted and treated in our hospital from December 2015 to December2016 were convenient selected and divided into two groups with 44 cases in each, the routine group were treated with routine plate, while the treatment group were treated with locking plate, and the treatment effect, changes of various indexes and occurrence probability of complications of the two groups were observed and compared. Results The total treatment effective rate in the treatment group was obviously higher than that in the routine group,（ 42 cases vs 34 cases, 95.45% vs77.27%）, and the operation time, intraoperative bleeding amount, ankle joint function score, fracture healing time, posteroanterior radiograph angulation and lateral projection angulation in the treatment group were better than those in the routine group, [（46.4±2.9）min,（40.8±4.7）mL,（82.3±4.5）points,（17.8±0.4） weeks,（1.5±0.6）°,（1.2±0.4）° vs（52.4±5.3）min,（76.2±5.4）mL,（72.6 ±3.8）points,（26.4 ±1.6）weeks,（4.2 ±0.9）°,（3.2 ±0.7）° ], and the occurrence probability of complications in the treatment group was obviously lower than that in the routine group,（3 cases vs 11 cases, 6.82% vs 25.00%）, and the difference was statistically significance（P〈0.05）. Conclusion The locking plate in treatment of distal tibial Pilon fracture can not only improve the treatment effect, but also reduce the occurrence of complications to a certain degree, which is conducive to the rapid recovery of patients.
