利培酮与奥氮平对精神分裂症急性期的疗效及临床影响因素研究

Research on Curative Effect and Clinical Influence Factors of Risperidone and Olanzapine in Treatment of Schizophrenia in the Acute Stage

目的对比研究利培酮与奥氮平对精神分裂症急性期应用的疗效以及临床影响因素。方法方便选取如皋市精神病防治医院2015年1月—2016年1月诊治的精神分裂症急性期患者100例随机分成利培酮组与奥氮平组,每组50例。利培酮组药物剂量范围在2~6 mg/d,奥氮平组药物剂量范围在10~20 mg/d。两组患者均采用阳性与阴性症状评定量表(PANSS)进行为期6周的观察,在治疗过程中对患者用药情况进行准确的记录。结果 6周后,利培酮治疗精神分裂症急性期的有效率为86%,奥氮平治疗精神分裂症急性期的有效率为86%;奥氮平组患者在阳性症状与一般病理症状中的治疗效果明显好于利培酮组(P<0.05);随着治疗时间的增加,两种药物两组患者精神分裂症急性期的治疗效果均明显增加(P<0.05);治疗时间这一因素与利培酮和奥氮平两种药物不存在交互作用(P>0.05);治疗时间、剂量、病程对利培酮的疗效有明显影响(R_2=0.56,P<0.05),治疗时间对奥氮平的疗效有影响(R_2=0.38,P<0.05)。结论对精神分裂症急性期患者的治疗,利培酮与奥氮平疗效相当,但是奥氮平治疗效果的稳定性好于利培酮。

Objective To compare and research the curative effect and clinical influence factors of risperidone and olanzapine in treatment of schizophrenia in the acute stage. Methods 100 cases of schizophrenia patients in our hospital from January 2015 to January 2016 were convenient randomly divided into the risperidone group and olanzapine group with 50 cases in each, the drug dose range was 2 ~6 mg/d in the risperidone group, while the drug dose range was 10 ~20 mg/d in the olanzapine group, both groups were observed for six weeks by the PANSS, and medication of patients was accurately recorded in treatment course. Results After six weeks, the effective rates of risperidone and olanzapine in treatment of schizophrenia in the acute stage were respectively 86% and 86%, and the treatment effect in the positive symptoms and general pathological symptoms in the olanzapine group was obviously better than that in the risperidone group, and treatment effect obviously increased with the increase of time（P〉0.05）, and there was no interaction between the treatment time and risperidone and olanzapine（P〈0.05）, and treatment time, dose, treatment course had an obvious effect on risperidone（R2=0.56,P〈0.05）,and the treatment time had an effect on the curative effect of olanzapine（R2=0.38,P〈0.05）. Conclusion The curative effect of risperidone and olanzapine in treatment of schizophrenia in the acute stage is similar, but the stability of treatment effect of olanzapine is better than that of risperidone.
