Wellens Syndrome: a Case Report and Literature Review

Wellens综合征是一组以心电图T波对称性深倒置或正负双向性改变或演变为主,通常不伴ST-T改变的高危不稳定型心绞痛。Wellens综合征典型表现为胸痛发作时心电图正常,在胸痛缓解期间,心电图V_2~V_5导联T波呈对称性倒置或正负双向性改变,极易导致漏诊、误诊。

Wellens syndrome is an electrocardiographic manifestation of symmetrically and deeply inverted T waves or biphasic T waves evolution or changes,usually without ST-T wave changes in patients with high-risk unstable angina pectoris. It is easily missed diagnosed or misdiagnosed for usually during the attack of chest pain,the ECG is normal,while during the remission of chest pain,symmetrically inverted or biphasic T waves are found in leads V_2-V_5. Based on the review of the diagnosis and treatment of one case of Wellens syndrome in accordance with the 24 h dynamic ECG monitoring results and related literature,we elaborated the concept,diagnostic criteria,generative mechanism and clinical significance of Wellens syndrome,in order to improve the clinical understanding of this disease,by which the occurrence of missed diagnosis and misdiagnosis of Wellens syndrome can be avoided.
