晚期胃癌肝转移规范化治疗策略

Standardized treatment strategy of advanced gastric cancer with liver metastasis

肝脏作为晚期胃癌血行转移最主要的靶器官,是造成晚期胃癌病人死亡的主要原因。目前,肝切除仍是治疗肝转移的首选方式,但由于胃癌肝转移发生时常合并肝外病变尤其是腹膜转移和远处淋巴结转移,难以行根治性手术切除。因此,通常须考虑病人的一般情况,结合原发灶的病理学类型和肝脏的转移程度,采取个体化原则,实施综合治疗。

Liver is the main target organ in the blood metastasis of advanced gastric cancer. Currently, liver metastases is the main cause of death in patients with advanced gastric cancer. Liver resection is still the preferred treatment to cure liver metastasis from gastric cancer. Most patients have lost the chance of radical resection because of the patients with liver metastases from advanced gastric cancer usually merged with serious extrahepatic lesions, such as peritoneal metastasis and lymph node metastasis. Therefore, it is usually necessary to take a holistic treatment plan, which combined with the general situation of patients, the pathological types of primary tumor and the degree of liver metastasis.
