摘要
目的探讨实时双平面成像(Bi-plane)技术在胎儿心脏结构筛查的临床应用价值。方法选择150例孕妇作为研究对象,年龄22~36岁,平均年龄27.7岁;孕周24~30周,平均孕周26.2周。分别采用常规二维超声和实时Biplane技术两种方法成像主动脉弓切面、动脉导管弓切面、腔静脉长轴切面、右心室流出道切面、三血管-气管切面及二、三尖瓣短轴切面等基础切面,记录成像率和整体成像时间,并进行对比。结果常规二维超声法获取到的主动脉弓切面、动脉导管弓切面、右心室流出道切面及三血管-气管切面的成像率与实时Bi-plane技术的成像率比较,差异均无统计学意义(P>0.05)。常规二维超声法获取到的腔静脉长轴切面和二、三尖瓣短轴切面的成像率与实时Bi-plane技术的成像率比较,差异均有统计学意义(P<0.05)。常规二维超声法成像时间与实时Bi-plane技术比较,差异无统计学意义(P>0.05)。结论实时Bi-plane技术是一种产前观察胎儿心脏直接、准确、方便、快捷的新方法 ,在对胎儿心脏基础切面及信息检查方面具有独特的优势,可作为常规二维超声心动图的辅助检查方法,具有重要的临床应用价值。
Objective To explore the application value of real-time Bi-plane image technology in routine screening of fetal heart. Methods A total of 150 cases of pregnant women were enrolled, which aged 22-36 years old with mean age of 27.7years old; the gestational was 24-30 weeks with mean of 26.2 weeks. The aortic arch view, ductal arch view, vena cava long axis view, right ventricular outflow tract view, three-vessel-trachea view, mitral and tricuspid short axis views were visualized by routine two-dimensional echocardiography and real-time Bi-plane image technology, respectively. The visualization rates and acquisition time were recorded and compared between 2 technologies. Results There were no significant differences in visualization rates of aortic arch view, ductal arch view, right ventricular outflow tract view and three-vessel-trachea view between 2 technologies(P〉0.05). There were significant differences in visualization rates of vena cava long axis view and mitral and tricuspid short axis views between 2 technologies(P〈0.05). There were no significant differences of acquisition time between 2technologies(P〉0.05). Conclusion It is demonstrated that real-time Bi-plane image technology is directly, accurately, conveniently and quickly in observing fetal heart prenatally. It is showed that unique advantages in fetal cardiac routine view and information examination could be used as an important auxiliary examination of conventional two-dimensional fetal echocardiography with important clinical value.
