摘要
目的:探讨甲泼尼龙联合人血清蛋白对病毒性脑炎患儿血清及脑脊液中SP-D和Gal-9的影响。方法:选择2014年3月至2015年2月在我院治疗的病毒性患儿148例,根据随机数字表分为对照组和观察组,每组74例。对所有患儿进行基础治疗和更昔洛韦抗病毒治疗,对照组患儿采用甲泼尼龙静脉滴注,观察组患儿采用甲泼尼龙联合人血清蛋白进行治疗。对两组患儿临床疗效、血清及脑脊液SP-D和Gal-9水平进行评价和测定。结果:治疗前两组患儿的各指标均无统计学差异(P>0.05)。2个疗程后两组患儿的巴氏指数均显著提高(P<0.05),且观察组显著高于对照组(P<0.05);观察组患儿住院时间、退热时间和止吐时间均显著短于对照组(P<0.05);观察组和对照组的有效率分别为93.24%、81.08%,差异有统计学意义(P<0.05);两组患儿血清和脑脊液中SP-D水平均显著升高(P<0.05),且观察组显著高于对照组(P<0.05);两组患儿血清和脑脊液Ga1-9水平均显著降低(P<0.05),且观察组显著低于对照组(P<0.05);两组患儿的临床不良反应皆轻微且无显著差异(P>0.05)。结论:甲泼尼龙和人血清蛋白联合使用能有效治疗病毒性脑炎患儿,具有较高的临床推广价值。
Objective: To investigate the effect of methylprednisolone combined with human serum protein on SPD and Gal-9 in serum and cerebrospinal fluid of children with viral encephalitis.Methods: 148 children with viral encephalitis treated in our hospital from March 2014 to February 2015 were randomly divided into two groups,74 cases in each group.All children were treated with basic treatment and antiviral treatment with ganciclovir.Additionally,the children in the control group were treated with methylprednisolone,the children in the observation group were treated with meprednisone combined with human serum protein.The Barthel score,SP-D and Gal-9 levels of the two groups were measured to compare the clinical curative effects.Results: The Barthel score of two groups showed no significant difference before treatment (P〉0.05),and they were all increased after treatment (P〈0.05) and observation group's score was significantly higher than that of control group (P〈0.05); The hospitalization time,cooling time and antiemetic time in observation group were significantly shorter than those in the control group (P〈0.05); Effective rate in the observation group was 93.24%,which was significantly higher than that in the control group (81.08%) (P〈0.05); After treatment,and SP-D levels in serum and cerebrospinal fluid of two groups were significantly higher than those before treatment (P〈0.05),and SP-D levels in serum and cerebrospinal fluid in observation group were significantly higher than those in the control group (P〈0.05).After treatment,the Ga1-9 levels in serum and cerebrospinal fluid of two groups were significantly lower than those before treatment (P〈0.05),and the Ga1-9 levels in serum and cerebrospinal fluid in observation group were significantly lower than those in the control group (P〈0.05).The adverse reaction of two groups was slight and showed no significant difference (P〉0.05).Conclusion: Methylprednisolone combined with human serum protein is more effective in the treatment of viral encephalitis,which has high clinical value.
