消痞镇痛膏外敷联合奥施康定片治疗肺癌重度疼痛30例临床观察

Clinical Observation on 30 Cases of Severe Pain of Lung Cancer Treated by External Application of Xiaopi Zhentong Gao(消痞镇痛膏) Combined with Oxycontin Tablets

目的观察消痞镇痛膏外敷联合奥施康定片治疗肺癌重度疼痛的临床疗效及安全性。方法 60例肺癌重度疼痛患者随机分为治疗组和对照组各30例。治疗组予消痞镇痛膏外敷联合奥施康定片口服;对照组予安慰剂外敷联合奥施康定片口服治疗,给药7天。观察两组患者治疗前后疼痛评分、奥施康定片日均消耗量、免疫功能指标及便秘伴随症状积分,并评价疼痛疗效。结果治疗组治疗后疼痛缓解率为90.0%,高于对照组的76.7%(P<0.05);治疗组治疗后奥施康定片日均消耗量少于对照组(P<0.05);两组患者治疗后免疫指标较治疗前均有所提高,且治疗组优于对照组(P<0.01);两组患者治疗后卡氏评分较治疗前有所提高(P<0.01),但治疗后两组卡氏评分比较差异无统计学意义(P>0.05);治疗组的副反应发生率明显少于对照组(P<0.05),且治疗组便秘伴随症状积分明显低于对照组(P<0.01)。结论消痞镇痛膏与奥施康定片有协同、减毒、增效的作用,能有效的减轻癌痛,减少奥施康定片的日均消耗量及副反应,改善患者免疫功能,提高患者生活质量。

Objective To observe the clinical efficacy and safety of External Application of Xiaopi Zhentong Gao （消痞镇痛膏, XZG） combined with Oxycontin tablets in the treatment of severe pain of lung cancer. Methods Sixty patients suffering from severe pain of lung cancer were randomized into treatment group and control group with 30 cases in each group. The treatment group was given external application of WZG combined with oral Oxycontin tablets, while the control group was given external application of placebo combined with oral Oxycontin tablets. Both groups were treated for 7 days. The pain score, daily consumption of Oxycontin tablets, immune function index and constipation associated symptom score were observed before and after treatment, and the effect for pain was evaluated. Results The remission rate of pain in the treatment group was 90.0%, higher than that in the control group （76. 7%, P 〈0. 05）. After treatment, the daily consumption of Oxycontin tablets in the treatment group was lower than that in the control group （P 〈 0. 05 ）. After treatment, the immune indexes of both groups improved compared with those before treatment, and the treatment group was superior to the control group （P 〈0.01 ） ; Karnofsky score in both groups significantly improved （P 〈 0.01 ） after treatment, but there was no significant difference between groups （P 〉 0. 05 ） ; the incidence of side effects in the treatment group was significantly lower than that in the control group （P 〈 0. 05 ）, and the constipation associated symptom score in the treatment group was significantly lower than that in the control group （P 〈 0.01 ）. Conclusion XZG seems have synergistic effects with Oxycontin tablets, enhancing its effect and reducing its toxicity. XZG could effectively reduce cancer pain, reduce the daily consumption of Oxycontin tablets and the side effects, improve the immune function of patients and improve their quality of life.
