摘要
目的探讨研究雄激素受体(AR)与黑色素瘤分化相关基因9/配体蛋白聚糖结合蛋白(Mda9/syntenin)在三阴性乳腺癌中的表达及其意义。方法 80例三阴性乳腺癌组织,采用免疫组织化学法对其AR与syntenin的表达情况检测,并探讨二者与临床病理特点的关系,采用Kaplan-Meier生存曲线评价患者无病生存率,COX比例回归模型进行多变量分析。结果 AR阳性表达率为37.50%,syntenin阳性表达率为98.75%;AR与组织学分级呈负相关性(γs=-0.236,P<0.05),syntenin与淋巴结转移状态呈正相关性(γs=0.296,P<0.05),与肿瘤大小呈正相关性(γs=0.370,P<0.05),与组织学分级也呈现正相关性(γs=0.230,P<0.05);无病生存者占77.5%(62/80),AR阴性与AR阳性患者、不同程度AR表达程度与无病生存期(DFS)均无相关性(P>0.05),而syntenin不同表达程度与DFS存在相关性(P<0.05);单因素分析显示:淋巴结转移是影响DFS的重要因素(P<0.05),多因素分析显示淋巴结转移、肿瘤>5 cm均是DFS危险因素(P<0.05)。结论 AR与患者的组织学分级相关、syntenin与组织学分级、淋巴结转移情况、肿瘤大小呈正相关性,syntenin表达与DFS存在明显关联。
Objective To study the expression and significance of androgen receptor (AR) and ligand binding protein (Mda9/syntenin) in triple negative breast cancer. Methods Expression of AR and Mda9/syntenin in 80 cases of triple negative breast cancer tissues were detected by immunohistochemical method, and the relationships with clinical pathological characteristics were analyzed. The Kaplan-Meier survival curve was used to evaluate the disease free survival rate of patients, and the COX pro- portional regression model were used to carry out multivariate analysis. Results The positive expression rate of AR was 37.50% ,and the positive expression rate of syntenin was 98.75%. The expression of AR was negatively related with histological grading (γs = - 0. 236, P 〈 0.05 ), while the expression of syntenin was positively related with lymph node metastasis ( γs = 0. 296,P 〈0.05 ), and positively related with tumor size (γs =0. 370,P 〈 0.05 ), as well as with histological grading (γs = 0. 230 ,P 〈0.05). The disease free survival accounted for 77.5% (62/80). There had no significant differences in disease free survival (DFS) between the AR negative and AR positive patients, and different degrees of AR expression (P 〉 0.05 ), while the different degrees of syntenin expressions were significantly correlated with DFS ( P 〈 0.05 ). The single factor analysis showed that lymph node metastasis was an important factor affecting the DFS ( P 〈 O. 05 ) , and the multi-factor analysis showed that both lymph node metastasis and tumor larger than 5cm were risk factors affecting DFS ( P 〈0. 05 ). Conclusion The expression of AR is related with histological grading, and the expression of syntenin is positively related with histological grading,lymph node metas- tasis and tumor size, which is obviously related with DFS.
