派丽奥与牙康局部治疗慢性牙周炎的系统评价

Efficacy of periocline and Yakang in the treatment of chronic periodontitis: a meta-analysis

目的:采用Meta分析方法探讨在牙周基础治疗后,局部使用派丽奥(2%盐酸米诺环素软膏)与牙康(甲硝唑棒)治疗慢性牙周炎的临床疗效。方法:计算机检索Pub Med、OVID、CNKI、CBM和Wan Fang Data数据库、维普数据库等。按照纳入、排除标准选择文献,查找基础治疗后,局部使用派丽奥与牙康治疗慢性牙周炎的随机对照试验(RCT),并追溯纳入文献的参考文献,收集日期截止至2016年12月。按照纳入和排除标准进行文献筛选、资料提取、评价纳入研究的方法学质量后,采用Review Manager 5.3软件进行Meta分析。结果最终纳入8个RCT,859例受试者。结果:亚组分析结果显示,对于探诊深度大于5 mm及小于3 mm患者,局部使用派丽奥在探诊深度、附着丧失、牙龈出血指数等方面差异有统计学意义,对于探诊深度介于3 mm至5 mm之间患者,尚不能确定两种药物是否有差异。结论:对于探诊深度大于5 mm及小于3 mm患者,在牙周基础治疗后采用派丽奥局部治疗与采用牙康局部治疗相比,效果更佳,更值得推荐。

Objective： To investigate the efficacy of local administration of periocline （2% minoCycline hydrochloride ointment） and Yakang （metronidazole stilus） after conventional periodontal therapy in the treatment of chronic periodontitis by a meta-analysis. Methods： The randomized controlled trials （RCT） of local administration of periocline and Yakang after conventional periodontal therapy in the treatment of chromic periodontitis published by December 2016 were collected by a computerized search of PubMed, OVID, CNKI, CBM, Wanfang Database, and VIP Database. Literature was screened according to the inclusion and exclusion criteria, data were extracted, and the quality of enrolled studies was evaluated. The Review Manager 5.3 software was used to perform the meta-analysis. Finally, 8 RCTs involving 859 patients were em＇olled in this study. Results：According to the results of subgroup analyses, for patients with a probing depth greater than 5 mm or less than 3 mm, local administration of periocline achieved significantly better clinical outcomes in probing depth, attachment loss, and gingival bleeding than Yakang for patients with a probing depth between 3 mm and 5 ram. However, there were no significant differences in clinical outcomes between these two drugs. Conclusions： Due to better treatment outcomes than Yakang, local administration of periocline after conventional periodontal therapy is recommended for patients with a probing depth greater than 5 mm or less than 3 ram.
