类风湿性关节炎肺内渐进坏死性结节的HRCT表现及诊断价值

HRCT Findings and the Diagnosis Value of the Lungs Gradual Necrotic Nodules in Rheumatoid Arthritis Patients

利用HRCT对类风湿性关节炎肺内渐进坏死性结节进行影像学诊断,指导临床治疗。回顾性分析20例经组织病理学或临床证实的类风湿性关节炎肺内渐进坏死性结节,评估其大小、位置、形态及间接征象的HRCT影像学表现特征。结果 20例患者共48个病灶大小分布于0.4cm^5.4cm之间,多分布于胸膜下,边缘不光滑;中心可伴有空洞形成;可同时伴有肺间质纤维化或难治性气胸等并发症。故类风湿性关节炎肺内渐进坏死性结节有独特的HRCT表现,充分认识并确定其特征性改变,有助于提高诊断的准确率。

Retrospective analysis of 20 cases of histopatho logy or clinically proven rheumatoid arthritis lung progressive necrosis no dules to assess its size, lo cat ion , shape and indirect signs of HRCT imaging characteristics. 20patientsw ere divided into 48 lesions in the range of 0. 4cm- 5. 4cm mostly distributedsmooth; the center may be associated with empty fo rmation; can also be ac companied by pulmo nary interstitial fibrosis or refractory pneumothorax and other concurrent disease.
