1例扩张型心肌病合并心力衰竭患者的药学监护

Pharmaceutical care on a patient with dilated cardiomyopathy and heart failure

1例44岁男性患者,因胸闷、乏力入院,给予强心、利尿、延缓心肌损伤、营养心肌、降尿酸等对症支持治疗。住院期间临床药师对患者进行药学监护,包括药物重整、心衰治疗方案的优化、延缓心肌损伤药物的建议、血压、心率、电解质、肝肾功能、左西孟旦、中成药、高尿酸血症的监护,以及对患者进行用药教育,并根据监护结果调整治疗方案,最终患者好转出院。

One 44-year-old male patient with chest tightness and fatigue was hospitalized.The patient was given symptomatic treatment including cardiac drugs,diuretics,delaying myocardial damage,myocardial nutrition support therapy and decreasing uric acid therapy.Pharmaceutical care was given to the patient by clinical pharmacist,such as drug reforming,optimization of medication regimen for heart failure,suggestion to delay myocardial injury,rational use of levosimendan and traditional Chinese medicine,medication education,monitoring of blood pressure,heart rate,electrolyte disturbances,liver and kidney function,and urate.Therapeutic regimen was adjusted in time according to the monitoring results.And finally,the patient became better and discharged from hospital.
