摘要
1例49岁男性患者,右足恶性黑色素瘤手术切除后出现胸闷、胸痛,诊断为"急性肺血栓栓塞症",给予抗凝、抗感染及对症治疗。临床药师对患者进行全程药学监护,包括结合指南和专家共识评估病情分级、协助制定抗凝方案、根据监测指标调整抗凝剂量、监护不良反应、进行心理干预、指导患者学会自我管理。患者INR达到目标值,病情平稳后出院。
One 49-year-old male patient,who felt chest tightness and chest pain after resection of malignant melanoma of the right foot,was diagnosed as acute pulmonary embolism.The patient was given anticoagulant,anti-infective and symptomatic treatment.The clinical pharmacists carried out the whole course pharmaceutical care for the patient,including using guidelines and expert consensus to assess the disease status,develop anticoagulant regimens,adjust anticoagulant dosage according to monitoring index,monitor adverse drug reaction,conduct psychological intervention and instruct the patient to learn self-management.After the therapy,the patient reached target value of INR and was discharged with stable condition.
