胎盘多肽注射液致迟发型过敏反应1例

One case of delayed allergy caused by placental polypeptide injection

1例84岁老年女性患者,因胰腺癌入院行伽玛刀治疗。治疗期间给予胎盘多肽注射液(8 mL,qd,ivgtt)增强免疫力,患者使用该药至第12天时输液10 min后出现颈部皮肤发红、胸闷、气短等反应,立即停止输注胎盘多肽注射液,给予持续吸氧(3 L·min^(-1)),地塞米松磷酸钠注射液(5 mg,iv),盐酸苯海拉明注射液(20 mg,im)等抗过敏治疗,约10 min后上述症状缓解。相关检查结果排除了心脏疾病的可能性。晚间患者多次出现胸部不适、气短,发作时胸部无明显阳性体征,未作特殊处理上述症状自行缓解。

One-84-year-old female patient diagnosed with pancreatic cancer was hospitalized for gamma knife therapy.The placental polypeptide injection（8 mL,qd,ivgtt） was given to the patient for enhancing immunity during the course of treatment.On the 12 th day of administration,the patient appeared redness of the neck skin,chest tightness,shortness of breath after infusion 10 minutes.Placental polypeptide injection was immediately stopped.And inhalation of oxygen（3 L·min^（-1））,dexamethasone sodium phosphate injection（5 mg,iv）,diphenhydramine hydrochloride injection（20 mg,im） were given to the patient.The patient became better gradually 10 minutes later.Heart disease was excluded by coherence check.Repeated chest discomfort,shortness of breath was occurred without obviously positive body signs at that night.And the above symptoms were self-mitigated without special treatment.
