经伤椎单侧置钉并椎体内植骨内固定治疗胸腰椎骨折临床观察

Clinical observation of injured vertebral unilateral nailing and vertebral graft internal fixation in treating thoracolumbarvertebral fracture

目的研究经伤椎单侧置钉联合椎体内植骨内固定治疗胸腰椎骨折的临床效果。方法将本院76例胸腰椎骨折患者根据手术方法不同分为对照组38例,行椎体后外侧短节段椎弓根钉植骨内固定术治疗;研究组38例,行经伤椎单侧置钉联合椎体内植骨内固定术进行治疗。比较2组患者影像学指标及治疗效果。结果治疗后2组患者伤椎椎体高度比明显增加,Cobb角、上位椎体楔形变角明显下降,但对照组改善程度低于研究组,且术后对照组患者VAS、ODI评分下降程度低于研究组,差异有统计学意义(P<0.05)。结论采用经伤椎单侧置钉联合椎体内植骨内固定治疗胸腰椎骨折效果显著,可改善神经功能,促进腰椎功能恢复。

Objective To investigate the clinical effect of injured vertebral unilateral nailing and vertebral graft internal fixation in treating thoracolumbar vertebral fracture.Methods A total of 76 patients with thoracolumbar vertebral fracture in our hospital were selected and divided into a control group and an observation group, with 38 cases in each group. The patients in the control group were given vertebral body posterolateral short-segment posterior instrumentation and the patients in the observation group were given injured vertebral unilateral nailing and vertebral graft internal fixation. The imaging indicators and treatment effect of the two groups were compared. Results After the treatment, the injured vertebral body height ratio of the two groups obviously increased, and Cobb and upper vertebral body wedge variable angles obviously declined. However, the improvement degree of the control group was lower than that of the observation group and the descending degree of VAS and ODI score of the control group were lower than those of the observation group, and the difference was significant（P〈0.05）. Conclusion The application of injured vertebral unilateral nailing and vertebral graft internal fixation in treating thoracolumbar vertebral fracture has a significant effect. It can improve neurological function and promote the recovery of lumbar spine function.
