羟基磷灰石种植体涂层体内消减的相关研究

The analysis of the degradation of HA coating of a loaded implant

摘要 目的:分析羟基磷灰石(hydroxyapatite,HA)涂层种植体在人体内经过长时间服役后表面是否有涂层消减的现象。方法:收集1颗服役23年后因种植体周围炎拔除的HA涂层种植体,进行肉眼观察;并在种植体表面无牙结石部位选择位点进行表面分析,即利用扫描电子显微镜(scanning electron microscopy,SEM)观察种植体表面微形貌与X射线能谱仪(energy dispersive spectrometer,EDS)分析表面涂层成份。结果:肉眼观可见种植体表面未见涂层样结构,SEM显示为较均一的粗糙表面,偶见少量微凸起,EDS检测到凸起位点检出钙和磷元素:Ca(14.27%)和P(9.5%),其余区域仅检出微量Ca和P(1.17%和0.82%)或无Ca和P。结论:HA涂层种植体在罹患种植体周围炎的情况下,表面涂层存在消减现象。 Objective:To analyze the degradation of hydroxyapatite(HA) coating of a failed implant after long term function.Methods:The HA-coated implant functioning for 23 years was extracted due to peri-implantitis.The surface was analyzed by gross observation,scanning electron microscopy(SEM) for morphology,and energy dispersive spectrometer(EDS) for elements analysis respectively.Results:By gross observation and SEM test,no obvious coating-like structure was found,except for some micro-convexes.EDS analysis demonstrated that calcium and phosphorus were detected(14.27%,9.15%) at the convexes.For most of the areas,no or trace calcium and phosphorus(1.17%,0.82%) was found.Conclusion:The specific case analysis showed that HA coating of the implant almost degraded after the long term function in the human body.
