传统挂线法与分时挂线法治疗肛周脓肿的临床效果

Clinical effect of timesharing thread-drawing and conventional treatment on perianal abscess surgery

目的观察传统挂线法与分时挂线法治疗肛周脓肿的临床效果。方法选取行手术治疗的肛周脓肿患者78例，随机分为对照组38例和观察组40例，对照组采用传统的一期挂线引流方法治疗，观察组采用拔除引流管后再挂浮线的方法治疗，两组其他手术方法均相同。术后半年对两组患者进行电话随访，并运用统计学软件分析两组在临床疗效、围术期指标及术后并发症发生率等方面的差异。结果观察组临床总有效率为87.5%，对照组为76.3%，两组比较差异未见统计学意义（P〉0.05）；观察组住院时间[（13.6±1.9）d]及脓腔愈合时间[（20.4±1.6）d]均短于对照组[（22.1±2.1）、（28.5±1.3）d]；且观察组视觉模拟评分法（VAS）疼痛量表评分[（2.68±0.33）分]低于对照组[（5.98±0.31）分]，差异均有统计学意义（P〈0.05）；两组手术时间比较差异未见统计学意义（P〉0.05）；观察组术后并发症发生率均低于对照组，差异有统计学意义（P〈0.05）。结论传统一期挂线法与分时挂线法在肛周脓肿的治疗效果上相近，但分时挂线法具有恢复快、疼痛小且术后并发症发生率低等优点，值得临床大力推广。

Objective To study the effect of timesharing thread-drawing and conventional treatment on perianal abscess surgery. Methods Seventy-eight patients with perianal abscess surgery were divided into observation group （40 cases） and control group （38 cases） according to treatment methods. Patients in the control group were treated by the conventional thread-drawing way, but patients in the observation group were treated by drainage tube removing and float line banging, the operation method of the two groups was same. Then the differences of the two groups in therapeutic effect, perioperative indexes and postoperative complications were compared. Results The therapeutic effect of observation group was 87.5%, and the control group was 76.3% （P 〉 0. 05）. The hospital stay [ （13.6 ± 1.9）d] and pus cavity healing time [ （20. 4 ± 1.6 ） d ] of observation group were shorter than those of the control group [ （22. 1 ± 2. 1 ） d and （ 28.5 ± 1.3 ） d ], and the visual analogue scale （VAS） score of observation group （2.68 ± 0. 33 ） was lower than that of the control group （5.98 ± 0. 31 ） （ P 〈 0. 05 ）. The incidence of complications in the observation group was lower than that in the control group （P 〈 0. 05 ）. Conclusions There was no difference in curative effect between the two groups, but the timesharing thread-drawing has the advantages of quick recovery, less postoperative pain and lower incidence of com- plications, thus it is worthy of promoting.
