掌侧LCP钢板与桡背侧双钢板治疗桡骨远端C3型骨折的疗效分析

Clinical Effect of Volar LCP Volar plate and Radial Dorsal Double Plate in the Treatment of Type C3 Distal Radius Fractures

目的：比较分析掌侧LCP掌侧钢板与桡背侧双钢板治疗桡骨远端C3型骨折的临床疗效.方法：回顾分析2014年1月~2016年1月期间收治50例桡骨远端C3型骨折患者资料,随机分成2组.观察组行桡背侧双钢板内固定治疗,对照组行掌侧LCP掌侧钢板内固定治疗,记录2组手术时间、术中出血量、骨折愈合时间,术后复查X光线测量掌倾角、尺偏角及、桡骨高度,末次随访时Conney腕关节功能评分指标及术后并发症情况.结果：患者获随访12~14.5个月,平均随访时间12.5个月.观察组与对照组手术时间、手术出血量比较（P﹥0.05）,无统计学意义,观察组和对照组骨折愈合时间基本相同,比较差异无统计学意义（P﹥0.05）.术后根据X光线测量桡骨掌倾角、尺偏角及桡骨高度,观察组均优于对照组,有统计学意义（P﹤0.05）.末次随访时根据Conney评价腕关节功能,观察组优良率92.0%（23/25）,对照组优良率72.0%（18/25）,2组比较差异有统计学意义（P﹤0.05）.结论：桡背侧双钢板内固定桡骨远端C3型骨折明显优于掌侧LCP钢板,有利患者早期进行腕关节功能锻炼,能明显改善患者腕关节功能.

Objective： Objective to compare the clinical effect of volar LCP volar plate and radial dorsal double plate in the treatment of type C3 distal radius fractures. Methods： Review of the patient data analysis from January 2014 to January 2016, a total of S0 eases of type C3 fracture of the distal radius were randomly divided into two groups ： the observation group underwent radial dorsal double plate fix-ation and the control group underwent volar LCP plate fixation, to record the two groups of operation time, bleeding volume, fracture healing time, postoperative X ray measuring palmar tilt, ulnar deviation, radial height and, at the end of the follow - up Conney wrist function score and postoperative complications. Results： The patients were followed up for 12 to 14.5 months, with an average follow -up time of 12.5 months. The observation group and the control group in the operation time and the amount of bleeding were compared （ P 〉 0.05 ）, the difference was not statistical significance, the observation group and the control group in the healing time of fracture was basically the same, the difference was not statistically significant （ P 〉 0.05）. According to the postoperative X ray measuring radial palmar tilt, ulnar deviation and radial height, the observation group were better than the control group, was statistically significant （ P 〈 0.05）. At the last follow- up, according to Conney evaluation of wrist joint function, the excellent and good rate was 92% （23/25） in the observation group, the excellent and good rate of the control group was 72% （ 18/25 ）, the difference between the two groups was statistically signifi- cant （P 〈 0.05）. Conclusion： The treatment of C3 type fracture of the distal radius with radial double plate fixation is superior to the volar LCP plate, which is beneficial to the early functional exercise of the wrist, and can significantly improve the wrist function.
