化痰通络法针药并用治疗脑卒中后抑郁的临床研究

Clinical study on treatment of post-stroke depression by Phlegm Tongluo acupuncture and medicine

目的观察化痰通络法针药并用治疗脑卒中后抑郁的临床疗效。方法选取我院于2016年3月至2016年10月期间收治的88例脑卒中后抑郁患者为研究对象,按就诊编号分为两组,单号纳入对照组(n=44),双号纳入研究组(n=44)。所有患者均接受脑卒中常规治疗,对照组在常规治疗基础上服用抗抑郁西药帕罗西汀治疗,研究组在常规治疗基础上采用化痰通络法针药并用治疗。比较两组治疗前后神经功能缺损程度、抑郁程度的变化及不良反应发生情况,并对两组临床疗效进行评价。结果较治疗前,两组NIHSS评分、HAMD评分在治疗后均下降,且研究组下降幅度更大,差异具有统计学意义(P<0.05);研究组临床疗效、不良反应发生情况均优于对照组,差异具有统计学意义(P<0.05)。结论对脑卒中后抑郁患者采用化痰通络法针药并用治疗,可显著提高临床疗效,改善患者抑郁程度,促进神经功能的恢复,且安全无不良反应,优势显著,值得临床大力推广。

Objective To observe the clinical effect of phlegm resolving and collateral unblocking acupuncture of post-stroke depression. Methods Eighty eight post-stroke depression patients were selected from November 2015 to May 2016. All patients were divided into two groups according to the numbers：odd number was included in the control group（n = 44）and even number was included in the study group（n= 44）. All patients were treated with routine stroke,The control group was given regular treatment and antidepressant paroxetine. The study group was given regular treatment and eliminating phlegm andphlegm resolving and collateral unblocking acupuncture treatment. The changes of neurological deficits,the degree of depression and the occurrence of adverse reactions were compared between the two groups before and after treatment,and the clinical efficacy of the two groups was evaluated. Results The scores of NIHSS and HAMD in both the two groups were decreased after treatment,and the study group decreased more dramatically. The difference was statistically significant（P〈0.05）. The clinical curative effect and adverse reaction occurred in the study group were better than those in the control group,the difference was statistically significant（P〈0.05）. Conclusion Phlegm resolving and collateral unblocking acupuncture treatment can significantly improve depressive symptoms and promote the restoration of nerve function in post-stroke depression patients.
