摘要
目的 :观察脑血管病患者中无肌阵挛症状者的巨大SEP。方法 :对 2 16例急性脑血管病患者进行正中神经体感诱发电位 (SEP)检测 ,并以 4 3例 4 0岁以上健康人作为对照组。巨大SEP标准为 :①N2 0成分振幅 >5 μV ;②P2 5或N35成分振幅 >10 μV。 结果 :2 16例中巨大SEP者 2 7例 (12 5 % ) ,其中P2 5或N35及N2 0成分均增高者 18例 (6 6 7% ) ,P2 5或N35成分增高而N2 0成分正常 9例 (33 3% )。与对照组比较N2 0、P2 5、N35成分差异均有显著意义 (P <0 0 0 1、P <0 0 0 1、P <0 0 1)。结论 :脑血管病患者所呈现的无肌阵挛症状的巨大SEP的表现主要是N2 0及P2 5或N35成分波幅均增高 ,仅少数N2 0成分正常 ,这与皮质反射性肌阵挛的特征性P2 5或N35成分增高、而N2
Objective:To observe the giant somatosensory evoked potential (SEP) in cerebral vascular disease (CVD) patients without symptoms of myoclonus . Methods:Median nerve SEPs were determined in 216 patients with acute CVD and 43 normal subjects over 40 years old were served as controls . The criteria of giant SEP included one or two of the following . ①The amplitude of wave N20 is >5?μV ,② The amplitude of P25 or N35 is >10?μV .Results:The giant SEP was found in 27 patients (12.5%) out of the 216 ones. The increase of the amplitude of P25 ,N35 and N20 was found in 18 patients (66.7%),patients with the amplitude increase of P25 or N35 , and with normal amplitude of N20 was only 9(33.3%). A significant difference in N20 , P25 ,N35 was noted as compared with the control group . Conclusions:The giant SEP in acute CVD patients without symptoms of myoclonus mainly present with increase of amplitude of P25 or N35 and N20 components . It might be different from the SEP type with characteristic increase of the amplitude of P25 or N35 , but with normal N20 ,which are noted in patients with cortex-reflex myoclonus.
