慢性咳嗽中医病因病机及证候学规律的认识与探讨

The study and discussion on traditional Chinese medicine etiology,pathogenesis and syndrome rule of chronic cough

慢性咳嗽为呼吸科常见病症,为中西医临床研究热点之一。有关研究表明,中医药治疗慢性咳嗽有独特优势。结合既往相关研究,就其病因病机、证候学规律等方面进行了探讨。认为慢性咳嗽多属内伤咳嗽,病位主要在肺,兼及他脏,而与中焦脾胃尤为相关。其病因病机多为外感所伤、饮食起居失节、失治误治,又因脏腑内伤、祛邪不尽,风邪、寒饮、湿热等邪气留伏于内,复由外邪引动,邪犯于肺,肺气上逆而发,具有内外合邪致病的特点。其中风邪伏肺为共同病机,主要证候表现为风邪伏肺、湿热郁肺、肺阳亏虚、寒饮伏肺等。

Chronic cough is a common disease of respiratory department,which is one of the focus in both Chinese and western medicine clinical research. Studies have shown that Chinese medicine has unique advantages in the treatment of chronic cough. Based on the previous studies,etiology and pathogenesis,as well as syndrome rules are discussed in this paper. We think chronic cough is mostly the internal injury cough,the main disease location is lung,combined with other organs,especially relevant with the spleen and stomach in middle energizer. The causes are mostly external infection,imbalance of daily life,mistreatment or couldn＇t be treated in time,and pathogenesis is the wind evil,cold fluid,damp heat,etc staying in body because of internal injury in organs or not enough of eliminating pathogenic factors,they will violate lung when temped by exogenous evil,then cause the reverse flow of gas from lung,so it has the characteristics of causing by the combination of internal and external factors. ＂Wind lurking in lung＂is the common pathogenesis,the main syndrome manifest wind lurking in lung,damp heat limit lung,lung Yang deficiency,cold fluid lurking in lung,etc.
