腹腔镜辅助胃癌手术并发症临床分析研究

Clinical analysis of complications of laparoscopic assisted gastric cancer surgery

目的分析腹腔镜辅助胃癌手术的并发症发生情况。方法 100例腹腔镜辅助胃癌手术患者为研究对象,回顾性分析其治疗效果,根据并发症发生情况分为发生组(发生并发症)及未发生组(未发生并发症),分析并发症发生的影响因素。结果 100例患者经手术治疗后,20例发生并发症(发生组),占比20.00%,其中以淋巴漏最为常见,其次为吻合口瘘及肺部感染;80例未发生并发症(未发生组)。发生组住院时间、首次进食时间、肛门排气时间均长于未发生组,差异均具有统计学意义(P<0.05)。Logistic回归分析显示,住院时间、首次进食时间、肛门排气时间均是导致并发症发生的独立危险因素。结论导致腹腔镜辅助胃癌手术患者术后发生并发症的原因为住院时间、首次进食时间、肛门排气时间,对此需加强术后观察,从而促进病情恢复。

Objective To analyze the occurrence of complications of laparoscopic assisted gastric cancer surgery. Methods A total of 100 patients with laparoscopic assisted gastric cancer surgery as study subjects, and their treatment effect was retrospectively analyzed. They were divided by occurrence of complications into occurrence group（with complications） and non-occurrence group（without complications）. The influence factors of the complication was analyzed. Results After surgical treatment, there were 20 cases with complications（occurrence group） in 100 cases, accounting for 20.00%. Among them, lymph leakage is the most common, followed by anastomotic fistula and pulmonary infection. 80 cases had no complications（non-occurrence group） The occurrence group had longer hospitalization time, first feeding time and anal exhaust time than non-occurrence group, and their difference was statistically significant（P〈0.05）. Logistic regression analysis showed that hospitalization time, first feeding time and anal exhaust time were the independent risk factors of complications. Conclusion The causes of postoperative complications in laparoscopic assisted gastric cancer surgery are hospitalization time, first feeding time and anal exhaust time. Postoperative observation should be strengthened, so as to promote the recovery of the disease.
