远端胃癌腹腔镜D2根治术与传统开腹术的近期疗效对照观察

Comparative observation of short-term efficacy of laparoscopic D2 radical distal gastrectomy for gastric cancer and conventional laparotomy

目的探讨分析远端胃癌腹腔镜D2根治术与传统开腹术的近期疗效。方法 70例胃癌患者,依据患者入院时间的先后顺序分为研究组(30例)与对照组(40例)。研究组患者采用远端胃癌腹腔镜D2根治术治疗,对照组患者采用传统开腹术治疗。比较两组患者淋巴结清扫个数、切口长度、术中出血量、手术时间、肛门排气时间、住院时间以及并发症发生情况。结果研究组患者切口长度短于对照组患者,术中出血量少于对照组患者,手术时间、肛门排气时间、住院时间均短于对照组患者,差异具有统计学意义(P<0.05);两组患者淋巴结清扫个数比较差异无统计学意义(P>0.05)。研究组患者术后并发症发生率为6.67%,明显低于对照组患者的25.00%,差异具有统计学意义(P<0.05)。结论应用远端胃癌腹腔镜D2根治术治疗胃癌与传统开腹术比较,远端胃癌腹腔镜D2根治术的临床近期疗效更高,手术并发症发生率更低,值得临床推广及应用。

Objective To investigate the short-term efficacy of laparoscopic D2 radical distal gastrectomy for gastric cancer and conventional laparotomy.Methods A total of 70 gastric cancer patients were divided by admission order into research group（30 cases） and control group（40 cases）.The research group was treated with laparoscopic D2 radical distal gastrectomy for gastric cancer,and the control group was treated with conventional laparotomy.Comparison were made on number of lymph node dissection,length of incision,intraoperative bleeding volume,operation time,anal exhaust time,hospitalization time and incidence of complications between two groups.Results The research group had shorter length of incision than the control group,less intraoperative bleeding volume than the control group,and shorter operation time,anal exhaust time and hospitalization time than the control group.Their difference was statistically significant（P〈0.05）.Both groups had no statistically significant difference in number of lymph node dissection（P〉0.05）.The research group had obviously lower incidence of postoperative complications as 6.67% than 25.00% in the control group,and the difference was statistically significant（P〈0.05）.Conclusion Compared with conventional laparotomy,laparoscopic D2 radical distal gastrectomy for gastric cancer shows higher clinical short-term efficacy and lower incidence of surgical complications.It is worthy of clinical promotion and application.
