单核细胞CX3C趋化因子受体1表达与冠状动脉狭窄性疾病的关系 被引量:5

摘要 目的探讨单核细胞CX3C趋化因子受体1(CX3CR1)表达与冠状动脉狭窄性疾病的关系。方法选择冠状动脉血管狭窄≥50%患者70例、冠状动脉血管狭窄<50%患者23例与无冠状动脉病变的非冠状动脉粥样硬化性心脏病志愿者24例分别为狭窄组、不明显病变组与对照组。用流式细胞术测定单核细胞CX3CR1荧光表达强度,比较3组单核细胞CX3CR1表达水平,进行Spearman相关分析,多因素logistic回归分析,ROC曲线分析相关危险因素。结果狭窄组、不明显病变组、对照组CX3CR1荧光强度分别为4.94%(2.41%,6.58%)、2.02%(1.25%,3.57%)、1.68%(1.09%,2.24%),3组比较有统计学差异(Hc=31.025,P<0.01)。与对照组比较,狭窄组CX3CR1荧光强度明显升高,有统计学差异(Z=4.934,P<0.01)。TG和CX3CR1是冠状动脉狭窄性疾病发病的危险因素(P=0.048,P=0.026)。CX3CR1荧光强度在临界值为2.91%时,敏感性为64.1%,特异性为91.7%,曲线下面积为0.810,CX3CR1对冠状动脉狭窄病变有一定的诊断效力(95%CI:0.728~0.891,P<0.01)。结论单核细胞CX3CR1高表达是冠状动脉狭窄性疾病发病的危险因素,对冠状动脉狭窄性疾病诊断有一定的效力。 Objective To study the association between CX3 CR1 expression in monocytes and coronary artery stenosis(CAS).Methods Seventy patients with their CAS ≥50% served as a stenosis group,23 patients with their CAS 50% served as a non-significant lesion group and 24 volunteers with no CAS served as a control group.The CX3 CR1 fluorescence intensity in monocytes was detected by flow cytometry.The CX3 CR1 expression levels in 3 groups were compared.The related risk factors in CAS were analyzed by Spearman correlation analysis,multivariate logistic regression analysis and ROC curve analysis.Results The CX3 CR1 fluorescence intensity was4.94%(2.41%,6.58%),2.02%(1.25%,3.57%)and 1.68%(1.09%,2.24%)respectively in stenosis group,non-significant lesion group and control group(P〈0.01).The CX3 CR1 fluorescence intensity was singnificantly higher in stenosis group than in control group(P〈0.01).TG and CX3 CR1 were the risk factors for CAS(P=0.048,P=0.026).The area under the ROC curve was 0.810 when the cut-off value was 2.91%,the sensitivity was 64.1%,and the specificity was91.7%for the CX3 CR1 fluorescence intensity,indicating that CX3 CR1 plays a certain role in diagnosis of CAS(P〈0.01).Conclusion The CX3 CR1 expression level is significantly higher in CAS patients and is possibly involved in CAS.Elevated CX3 CR1 expression level in monocytes is a risk factor for CAS and plays a certain role in diagnosis of CAS.
