生肌象皮膏联合天降血栓通丸治疗臁疮疗效观察

Observation on the Clinical Effect of Shengjixiangpi Paste in Combination with Tianjiang Anti-thrombus Pill in the Treatment of Ecthyma

目的观察生肌象皮膏联合天降血栓通丸治疗臁疮的临床疗效。方法将2013年10月-2016年10月天津市北辰区中医医院外科收治的80例臁疮患者随机分为观察组(40例)与对照组(40例),对照组患者创面采用生肌象皮膏换药治疗,观察组患者在对照组治疗的基础上加服天降血栓通丸,对比观察两组患者的创面面积、疼痛评分、纤维蛋白原及D-二聚体含量的变化情况以及两组患者的创面愈合情况,结果治疗8周后,两组患者创面面积及疼痛评分对比,t=23.075及16.200,P均<0.01,差异具有统计学意义;纤维蛋白原及D-二聚体含量对比,t=4.533及7.273,P均<0.01,差异具有统计学意义。治疗8周后,观察组痊愈7例,显效21例,有效5例,无效7例,总有效率为82.5%;对照组痊愈4例,显效14例,有效4例,无效18例,总有效率为55.0%,两组对比,u=2.434,P<0.01,差异具有统计学意义。结论生肌象皮膏联合天降血栓通丸治疗臁疮,可有效促进创面愈合,缓解创面疼痛,疗效显著,值得临床推广应用。

Objective To observe the clinical effect of Shengjixiangpi paste combined with Tianjiang anti- thrombus pill in the treatment of ecthyma. Methods 80 patients with ecthyma admitted to the Surgery Department of Tradi- tional Chinese Medicine Hospital of Tianjin Beichen District were randomly divided into observation group （40 cases） and control group （40 cases）. Patients of control group were treated with Shengjixiangpi paste while patients of observation group were treated with Shengjixiangpi paste and Tianjiang anti-thrombus pill, followed by the observation on and compari- son of wound area, wound pain severity, treatment effective rate and content variation of fibrinogen and d-double polymer. Results After 8 weeks of treatment, the comparison of wound area and pain severity showed statistically significant differ- ence （ t＇= 23. 075 and 16. 200, P 〈 0. 01 ）. A comparison of content of fibrinogen and d-double polymer showed statistically significant difference （ t =4. 533 and 7. 273, P 〈 0. 01 ）. After 8 weeks of treatment, in observatirn group, 7 patients were cured and the therapy was markedly effective for 21 patients, effective for 5 patients and ineffective for 7 patients. The total effective rate was 82. 5%. In control group, 4 patients were cured and the therapy was markedly effective for 14 patients, effective for 4 patients and ineffective for 18 patients. The total effective rate was 55%. The comparison of the two groups showed statistically significant difference （ u = 2. 434, P 〈 0. 01 ）. Conclusion The combination of Shengjixiangpi paste and Tianjiang anti-thrombus pill can promote wound healing and relieve pain in the treatment of ecthyma, worthy of being promoted in clinical practice due to its remarkable clinical efficacy.
