参苡建中汤联合替吉奥治疗中晚期胃癌临床疗效及安全性分析

Clinical Analysis of Shenyi Jianzhong Decoction Combined with S-1 in Treatment of Advanced Gastric Cancer

目的:探讨参苡建中汤联合替吉奥化疗治疗中晚期胃癌的疗效及安全性,为临床治疗该病提供新思路。方法:收集西安市中医医院就诊的中晚期胃癌患者82例作为研究对象,随机分为治疗组和对照组各41例。对照组患者采用替吉奥进行化疗,治疗组在对照组基础上加用中药参苡建中汤口服。观察治疗前后两组患者免疫水平、临床疗效、中医症状及生存质量改善情况,评价不良反应。结果:治疗后,治疗组患者CD_3^+、CD_4^+T细胞数量以及CD_4^+/CD_8^+均显著高于治疗前,CD_8^+T细胞数量显著低于治疗前,差异均有统计学意义(P<0.05);对照组患者CD_3^+、CD_4^+T细胞数量高于治疗前(P<0.05),而CD_8^+T细胞数量及CD_4^+/CD_8^+与治疗前相比无显著差异(P>0.05)。组间比较,治疗后两组患者各免疫指标均有显著差异(P<0.05)。治疗后,治疗组患者有效率及疾病控制率分别为65.85%和82.93%,对照组患者有效率及疾病控制率分别为41.46%和60.98%。治疗组显著高于对照组,差异有统计学意义(P<0.05)。治疗前两组患者中医症状评分无显著差异,治疗后治疗组患者中医症状评分明显低于治疗前,同时明显低于对照组,差异均有统计学意义(P<0.05)。治疗后,治疗组患者生存质量有效率为82.93%,对照组为56.10%,治疗组显著高于对照组,差异有统计学意义(P<0.05)。治疗后,除两组患者脱发发生率无显著差异外,治疗组其他不良反应发生率均明显低于对照组,差异有统计学意义(P<0.05)。结论:参苡建中汤联合替吉奥化疗治疗中晚期胃癌具有良好的疗效,可有效减少化疗所致不良反应,对于改善患者生存质量具有积极的意义。

Objective： To explore the clinical effect and safety of Shenyi Jianzhong decoction combined with S-1 in treatment of advanced gastric cancer,in order to provide new insights for clinical treatment. Methods： 82 patients with advance gastric cancer accepted in Xi＇an hospital of traditional Chinese medicine were selected and randomly divided into the treatment group and the control group with 41 cases in each. S-1 was given to control group,and Shenyi Jianzhong decoction combined with S-1 were given to treatment group. Immunity level,total efficacy,TCM symptom score and life quality and adverse reactions of two groups were compared. Results： Immunity level of treatment group after treatment was obviously better than before and control group with statistically significance（ P〈 0. 05）. Total efficacy and disease control rate of treatment group were respectively 65. 85% and 82. 93%,of control group respectively 41. 46% and 60. 98%. There was statistically difference between two groups. TCM symptom score of treatment group after treatment was obviously better than before treatment and control group with statistically significance（ P 〈0. 05）. Life quality improvement rate of treatment group was 82. 93%,of control group was 56. 10%,which had statistically difference（ P 〈0. 05）. Adverse reaction rate of treatment group was obviously lower than control group（ P 〈0. 05）. Conclusion： Using Shenyi Jianzhong decoction combined with S-1 in treatment of advanced gastric cancer can reduce adverse reactions of chemotherapy with good efficacy,which has positive significance in improvement of life quality.
