慢性肾脏病2~5期患者血浆BNP水平与心功能关系的研究 被引量:2

摘要 目的探讨慢性肾脏病(CKD)2~5期患者血浆B性脑钠肽(BNP)水平以及其与心功能分级、脏左室舒张末期容积之间的相关性。方法收集CKD2~5期患者143例为研究对象,男82例,女61例,根据肾小球滤过率分为CKD 2、3、4、5期组,留取患者静脉血检测血浆BNP,用彩色多普勒超声心动图测定左室舒张末期容积。比较CKD各期患者血浆BNP水平,分析血浆BNP水平与心功能分级、左室舒张末期容积之间的相关性。结果 CKD2~5期患者中,随着CKD分期的进展,血浆BNP逐渐升高,左室舒张末期容积逐渐增加,差异有统计学意义。spearman相关性分析提示血浆BNP与心功能分级正相关,Pearson相关性分析提示,BNP与左室舒张末期容积正相关。结论在CKD2~5期患者中,随着CKD分期的进展血浆BNP水平升高,组间差异有统计学意义。血浆BNP水平与心功能分级、左室舒张末期容积具有良好的相关性,是评价CKD患者心功能的可靠生物学标记物。 Objective To investigate the level of plasma B-type brain natriuretic peptide( BNP) in patients with 2 to 5 stage chronic kidney disease( CKD) and its correlation with cardiac function classification and left ventricular end diastolic volume. Methods 143 subjects( 82 male,61 female) with 2 to 5 stage chronic kidney disease were divided into 4 groups according to estimated glomerular filtration rate( e GFR) : CKD 2,3,4 and 5. Venous blood was collected from patients to detect plasma BNP,and left ventricular end diastolic volume was examined by color Doppler echocardiogram. Plasma BNP levels of patients with CKD were compared,and correlations of plasma BNP level,cardiac function classification and left ventricular end diastolic volume were analyzed.Results In stage CKD2 ~ 5 patients,with the development of CKD staging,plasma BNP increased and left ventricular end diastolic volume increased gradually. Difference between the four groups was statistically significant. Spearman correlation analysis indicated that plasma BNP was positively correlated with heart function classification,and Pearson correlation analysis indicated that BNP was positively correlated with left ventricular end diastolic volume. Conclusion In CKD2 ~ 5 stage patients,plasma BNP level increased with the development of CKD staging and there was significant difference between the groups. Plasma BNP level is correlated with heart function classification and left ventricular end diastolic volume,which is a reliable biomarker to evaluate heart function of CKD patients.
