摘要
目的:探讨输卵管积水患者子宫内置节育器时,行输卵管栓塞术的技能以及可能性。方法:回顾分析了158例需要行IFV-ET治疗的患者,同时伴有宫腔粘连和输卵管积水,术前在妇科宫腔镜下,行宫腔粘连分离术,并且置入节育器,防止宫腔再次粘连。患者在宫腔内置入节育器时,行输卵管栓塞术。结果:宫腔粘连患者158例,均为两侧输卵管积水,宫腔内有节育器,节育器形态有圆形、V形和T形。158例患者均行两侧输卵管栓塞。栓塞后,HSG显示两侧输卵管栓塞满意,宫腔内节育器无移位。结论:患者宫腔内的节育器对操作有一定的影响,行输卵管选择性插管时,一定要谨慎,缓慢绕过节育器,避免对节育器的移位。输卵管栓塞效果十分满意,同时可以缩短宫腔粘连分离术后到胚胎移植的时间,防止宫腔再次粘连降低IVF-ET的成功率。
Purpose: To study the feasibility of fallopian tube embolization in patients with hydrosalpinx and intrauterine device. Methods: One hundred and fifth-eight patients with intrauterine adhesions and hydrosalpinx, who had been treated with IVF-ET, were enrolled. Under hysteroscopy, the intrauterine adhesions were separated, and intrauterine device was placed preoperatively. In patients with intrauterine device, their fallopian tube was embolized. Results: One hundred and fifty-eight patients were with intrauterine adhesion, hydrosalpinx and intrauterine device. Their intrauterine devices were in circle, V and T shape. After embolization, HSG showed that both sides of fallopian tube were embolized, and intrauterine device was not shifted. Conclusion: Patients with intrauterine device has certain influence on the operation. Catheterization must be with cautious to avoid shifting the intrauterine device. The effect of fallopian tube embolization is very satisfying. It also shortens the time from post operation to embryo transfer. And it prevents the intrauterine adhesions again, which will reduce tlle success rate of IVF-ET.
