冷凝集素是一种自身抗体,在最适温度3-4℃可以凝集自身红细胞[1];随着温度升高,冷凝集素与红细胞解离,当温度超过28-30℃冷凝集素与红细胞未能解离时,机体将受到免疫损害。正常人体内存在低亲和性非致病的冷凝集素,且大多为多克隆IgM性抗体,滴度通常小于1∶64。高滴度的冷凝集素在室温环境中往往会引起患者红细胞自发性凝集,
To analyze the influence of cold agglutinin on blood typing and cross-matching test,and find out corresponding solution.Micro glass bead column method,tube test,absorption elution test,antiglobulin test of Coombs and cross-matching test with Polybrene were applied to identify specific antibody,and PCR-SSP to confirm the exact blood type together with serologic pattern.Incubation at 37℃,no puzzle with blood typing and cross-matching.Incubation at 37℃,RBC washing at 37℃ and/or cold absorption at 4℃could resolve the discrepancy of blood typing which is mediated by cold agglutinin,also PCR-SSP may further affirm the serologic pattern.
