580例中青年大肠息肉的临床、内镜与病理分析

Clinical,endoscopic and pathological characteristics of colorectal polyps in young and middle-aged patients: An analysis of 580 cases

目的:探讨中青年患者(≤60岁)大肠息肉的临床、内镜与病理特点及其与癌变的关系。方法:选择2015年1月至2017年4月我院收治的580例中青年大肠息肉患者,分析不同年龄组患者息肉类型、大小、位置及癌化的检出率。结果:51~60岁年龄组大肠息肉检出率最高(P<0.05),男女各年龄组检出率差异无统计学意义(P>0.05);各年龄组左半结肠息肉检出率均高于其他部位(P<0.05);大肠息肉直径>2.0cm组癌变检出率高于直径小于2cm组(P<0.05);≤40岁患者炎性息肉检出率高(P<0.05),>41岁患者管状腺瘤检出率高(P<0.05);≤30年龄组各类型息肉均无癌变,>30岁年龄组绒毛状腺瘤癌变检出率高于其他息肉类型(P<0.05)。结论:中青年大肠息肉好发于左半结肠,高年龄组息肉检出率高;息肉直径>2.0 cm、绒毛状腺瘤患者大肠息肉癌变检出率高。

Objective：To explore the clinical,endoscopic and pathological characteristics of the large intestine polyps in middle and young patients（less than or equal to 60 years old） and to analyze the relationship between colorectal polyps and canceration. Methods：580 cases of young and middle-aged patients with colorectal polyps was selected in our hospital from January 2015 to April 2017,and analyzed the detection rate of polyp type,size,location and canceration in different age groups. Results：Detection rate of colon polyp peaked in the 51-60 years age group（P〈0.05）,and there was no significant difference in detection rate between men and women（P〉0.05）. The detection rate of left half colon polyps in all age groups was higher than that of other parts（P〈0.05）.The cancer detection rate of large intestine polyps with diametergreater than 2.0cm was higher than that of the large intestine with diameter of less than 2cm（P〈0.05）. The detection rate of inflammatory polyps was higher in patients younger than 40 years old（P〈0.05）,and the detection rate of tubular adenomas in patients older than 41 years was higher than those under 41 years of age（P〈0.05）. There was no cancer detection rate of polyps in the age group under the age of 30,and the detection rate of villous adenoma in the age group above the age of 30 was higher than that of other polyps（P〈0.05）. Conclusion：The polyps of middle and young patients are are more likely to locate in left colon,and People of older group have a higher detection rate.Patients with polyp diameter greater than 2.0 cm and villous adenoma have higher cancer detection rate of polyps.
