妊娠甲状腺功能减退症与妊娠期高血压疾病关系分析

Analysis of the Relationship between Hypothyroidism in Pregnancy and Hypertensive Disorder Complicating Pregnancy

目的:探讨妊娠甲状腺功能减退症与妊娠期高血压疾病关系分析。方法:将2016年1月~2017年1月在广州市从化区太平镇神岗卫生院、广州市从化区妇幼保健院产检发现的50例妊娠期高血压疾病孕妇作为观察组,将同期50例血压正常孕妇作为对照组,分别检测血清促甲状腺激素(TSH)、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、甲状腺过氧化物酶抗体(TPOAb)、血糖各指标、各血脂指标,比较两组产妇的妊娠甲状腺功能减退症发生率、各甲状腺激素及抗体水平、血糖各指标、血脂水平。结果:观察组TSH、FT3、FT4、TPO-Ab、FBG、2hPG水平与对照组相比有明显差异(P<0.05);观察组中妊娠期高血压妊娠甲减发生率为14.29%,子痫前期的发生率为22.73%,显著高于对照组的6%;观察组中妊娠期高血压TPO-Ab阳性率为10.71%,子痫前期为22.73%,显著高于对照组的4%;两组间有统计学差异(P<0.05);观察组妊娠期高血压疾病合并甲减者与妊娠期高血压疾病者TC、TG、HDL-C、LDL-C相比有明显差异(P<0.05)。结论:妊娠甲状腺功能减退症与妊娠期高血压疾病有密切关联,对妊娠结局有不良影响,其发生可能与血脂代谢异常有关。

Objective:To discuss the relationship between ypothyroidism in pregnancy and hypertensive disorder complicating pregnancy.Methods:50 patients with hypertensive disorder complicating pregnancy treated in a hospital from January 2016 to January 2017 were selected as the observation group,another 50 normotensive pregnant women at the same time were enrolled as the control group.The serum thyroid stimulating hormone(TSH),free three triiodothyronine(FT3),free thyroxine(FT4),thyroid peroxidase antibody(TPO-Ab),the index of blood glucose,and the blood lipid index were detected,then the incidence of hypothyroidism in pregnancy,thyroid hormone and antibody levels,blood sugar levels,and the blood lipid levels were compared.Results:The levels of TSH,FT3,FT4,TPO-Ab,FBG and 2 hPG in the observation group were significantly different from those in the control group(P<0.05);The incidence of pregnancy induced hypertension and hypothyroidism in the observation group was 14.29%,and the incidence of pre eclampsia was 22.73%,significantly higher than that of control group(6%);The positive rate of TPO-Ab of hypertensive disorder complicating pregnancy in the observation group was 10.71%,and the incidence of pre eclampsia was 22.73%,significantly higher than that of control group(4%);There was statistical difference between the two groups(P<0.05);There were significant differences in TC,TG,HDL-C,LDL-C of patients with hypertensive disorder complicating pregnancy patients with hypothyroidism and patients with pregnancy induced hypertension in the observation group(P<0.05).Conclusion:The hypothyroidism in pregnancy is closely related with hypertensive disorder complicating pregnancy,which has a bad effect on pregnancy outcome,and it may be associated with lipid metabolic abnormalities.
