骨水泥半髋置换加钢丝环扎术与动力髋螺钉治疗老年人股骨粗隆间骨折的疗效观察

Clinical Efficacy Investigate of Bone Cement Hemiarthroplasty Combined with Wire Cerclage on Intertrochanteric Fracture in Elderly Patients

目的:探讨骨水泥半髋置换加钢丝环扎术应用于老年股骨粗隆间骨折的效果。方法:以我院2015年1月至2015年6月间收治的老年粗隆间骨折患者60例为研究对象,随机分为置换组和常规内固定组,置换组采用骨水泥半髋置换加钢丝环扎术治疗,常规内固定组采用动力髋螺钉治疗,比较两组术后情况的差异。结果:置换组术后第一、第三及第六个月Harris评分显著高于常规内固定组,置换组患者术后完全负重时间和骨折愈合时间显著低于常规内固定组,其差异均有统计学意义(P<0.05)。结论:骨水泥半髋置换结合钢丝环扎术是治疗粗隆间骨折的有效手段,其相比于传统内固定方法,更有利于患者术后恢复。

Objective： To investigate the clinical efficacy of bone cement hemiarthroplasty combined with wire cerclage on inter-trochanteric fracture in elderly patients. Methods： A total of 60 elderly patients with intertrochanteric fracture were randomly divided into three group, 30 patients in each group. Patients in control group were treated by dynamic hip screw, and patients in FHR group were treated by bone cement half hip arthroplasty combined with wire cerclage. The Harris score at 1 month, 3 month, 6 month, 1 year after treatment, and recover state were compared. Results： The full weight bearing time and healing time in FHR group was signifi-cantly less than control group（P〈0.05）. The Harris score at 1 month, 3 month , 6 month after treatment was significantly better than con-trol group（P〈0.05）. Conclusion： Bone cement half hip arthroplasty combined with wire cerclage has a great clinical efficacy and worth for clinical application.
