糖尿病前期患者C反应蛋白和胰岛素抵抗与非酒精性脂肪肝相关性分析

Relationship Between C Reactive Protein and Insulin Resistance and Nonalcoholic Fatty Liver Disease in Patients with Prediabetic

目的:由于糖尿病前期和非酒精性脂肪肝(NAFLD)发相关性研究少,所以本研究探讨糖尿病前期患者胰岛素抵抗和C反应蛋白对NAFLD的发生和发展的的关系。方法:入选100例糖尿病前期患者根据B超结果,将患者分为NAFLD组(n=52)和非NAFLD组(n=48),通过对血脂指标、肝功指标、C反应蛋白和胰岛素抵抗指数的检测,分析糖尿病前期患者C反应蛋白和胰岛素抵抗指数与NAFLD之间的关系。结果:(1)NAFLD组在TG(P=0.034),LDL-C(P=0.023),谷草转氨酶(P=0.000)、谷丙转氨酶(P=0.000)、GGT(P=0.000),C反应蛋白(P=0.000)和胰岛素抵抗指数(P=0.000)均显著的高于和非NAFLD组:(2)胰岛素抵抗指数(OR=5.887,95%CI 2.672-12.970,P=0.000)和hs CRP(OR=4.689,95%CI 2.658-8.046,P=0.000)均为NAFLD的危险因素;(3)胰岛素抵抗指数和Hs CRP(r=0.295,P=0.017)呈正相关。结论:炎症反应和胰岛素抵抗作为独立的介质,将糖尿病前期和NAFLD组关联了起来,同时炎症反应和胰岛素抵抗相互关联。

Objective： there are few studies on the relation-ship between prediabetes and nonalcoholic fatty liver disease （ NAFLD）.So this study was to investigate the relationship be-tween insulin resistance and C reactive protein in the develop-ment of NAFLD in patients with prediabetic. Methods： 100 cases of prediabetes patients selected according to the ultra-sound results, patients were divided into the NAFLD group （52 cases） and non NAFLD group （ n=48）, by detection of serum lip-ids, liver function indicators, C-reactive protein and insulin re-sistance index , analysis of the relationship between G reactive protein and insulin resistance and NAFLD,in prediabetes pa-tients. Results： ？NAFLD group were significantly higher than that of and non NAFLD group,in the TG （ P=0.034）, LDL-C （ P= 0.023）, gLutamic oxaloacetic transaminase （ P=0.000）, glutamic pyruvic transaminase （ P=0.000）, gamma glutamyl transferase （ GGT） （ P=0.000）, G reactive protein （ P=0.000） and insulin re-sistance index （ P=0.000） ： ？Insulin resistance index （2.672- 12.970 P=0.000, OR=5.887,95%CI） and HsCRP （2.658-8.046 OR二4.689,95%CI, P二0？000） were the risk factors 〇￡ NAFLD;③ Insulin resistance index and HsCRP （ r=0.295, P=0.017） were positively correlated. Conclusion： the C reactive protein and insulin resistance are independent mediators, which are associ-ated with the prediabetic and the NAFLD , and the correlation between the C reactive protein and insulin resistance.
