孕妇鼾症与妊娠期高血压病和先兆子痫的相关性分析

Association of snoring with pregnancy-induced hypertension and preeclampsia

目的分析孕妇在不同妊娠期鼾症的发生情况及其与妊娠期高血压病和先兆子痫发病的相关性。方法选择2016年7月至2016年12月我院妊娠的2 000例孕产妇在产后1天进行问卷调查,内容包括一般信息、妊娠期睡眠情况、妊娠期母体结局,单因素分析比较不同妊娠期鼾症组和无鼾症组妊娠期高血压病和先兆子痫的发病率差异,采用多因素logistic回归分析妊娠期高血压病和先兆子痫发病的危险因素。结果 2 000例孕妇中孕前鼾症者129例(6.45%),孕期新发鼾症者340例(17.00%),其中孕早期鼾症者29例(1.45%),孕中期鼾症者200例(10.00%),孕晚期鼾症者111例(5.55%)。2 000例患者随访期间出现妊娠期高血压病者178例(8.9%),其中先兆子痫者98例(4.9%)。在妊娠的不同分期中,鼾症组妊娠期高血压病和先兆子痫的发病率显著增加(P<0.05);多因素分析结果显示年龄(P=0.012,0.034)、孕前体重指数(P=0.008,0.015)、孕中期鼾症(P=0.035,0.016)和孕晚期鼾症(P=0.042,0.028)是妊娠期高血压病和先兆子痫发病的独立危险因素。结论随着妊娠的进展,孕妇鼾症的总发生率逐渐增高,妊娠中、晚期鼾症是妊娠期高血压病和先兆子痫的独立危险因素。

Objective： To explore the association of snoring with pregnancy-induced hypertension and preeclampsia.Methods：A total of 2 000 pregnant women in our hospital from July 2016 to Dec 2016 were selected for postnatal questionnaires one day after delivery.The survey included general information,gestational sleep and gestational maternal outcome.The incidence of hypertension and preeclampsia in different gestational period were compared between snoring group and non-snoring groups by univariate analysis.The risk factors associated with pregnancy-induced hypertension and preeclampsia were also analyzed by multi logistic analysis.Results：There were 129（6.45%）pre-pregnancy snoring and 34 new snoring during pregnancy（17.00%）in 2 000 pregnancy women.The incidence of snoring at 1^st,2^nd and 3^rd trimester were 1.45%（29/2 000）,10.0%（200/2 000）and 5.55%（111/2 000）respectively.Of the 2 000 patients,178（8.9%）had pregnancy-induced hypertensive and 98（4.9%）had preeclampsia.The occurrence of pregnancy-induced hypertension and preeclampsia in snorers was significantly higher than that in non-snorers（P〈0.05）.Multi logistic analysis showed that age（P=0.012 0.034）,BMI before pregnancy（P=0.008,0.015）,snoring at 2^nd trimester（P=0.035,0.016）and 3^rd trimester（P=0.042,0.028）were the significant risk factors for pregnancy-induced hypertension and preeclampsia.Conclusions：With the progress of pregnancy,the incidence of snoring was gradually increased,and the snoring at 2^nd trimester and 3^rd trimester were the significant risk factors for pregnancy-induced hypertension and preeclampsia.
