Xpert Mtb/RIF实验室检查在结核病诊断中的价值研究

Clinical Valuation of Xpert Mtb/RIF Detection Test for Diagnosis of Tuberculosis

目的:探讨利福平耐药实时荧光定量核酸扩增技术(Xpert Mtb/RIF)对结核病的诊断价值。方法:261例疑似结核病患者作为研究对象,均进行Xpert Mtb/RIF和痰涂片实验室检查,以影像学检查联合细菌学检查进行确诊,比较两组检查方法的诊断价值。结果:Xpert Mtb/RIF检查准确率(198例,75.86%)明显高于痰涂片检查(160例,61.30%),差异有统计学意义(P<0.05);并且Xpert Mtb/RIF检查的灵敏度(69.04%)、特异度(96.88%)、约登指数(0.659)、阳性预测值(98.55%)和阴性预测值(50.41%)均高于痰涂片检查(分别为51.27%、92.19%、0.435、95.28%和38.06%)。结论:Xpert Mtb/RIF检查方法对结核病诊断具有较高价值,有助于结核病的早期诊断和治疗。

Objective： To explore the diagnostic valuation of Xpert Mtb/RIF detection test for tuberculosis. Methods： 261 patients of tuberculosis were selected as the object of study. All patients were examined by Xpert Mtb/RIF and sputum smear detection test, and the imaging examination combined bacteriological examination was diagnosed with tuberculosis. The diagnostic valuation of Xpert Mtb/RIF and sputum smear detection test was compared. Results：The accuracy of Xpert Mtb/RIF detection test （198 cases, 75.86%） was significantly higher than sputum smear （160 cases, 61.30%） for tuberculosis diagnosis （P〈O.05）. The sensitivity （69.04%）, speci- ficity （96.88%）, Youden index （0.659）, positive predictive value （98.55%） and negative predictive value （50.41%） of Xpert Mtb/RIFdetection test were higher than sputum smear （51.27%, 92.19%, 0.435, 95.28% and 38.06% respectively）. Conclusion： Xpert Mth/ RIF detection test has a high diagnostic valuation for tuberculosis, helping the early diagnosis and treatment of tuberculosis.
