奥沙利铂联合替吉奥胶囊治疗晚期原发性肝癌的疗效观察

To Observe the Curative Effect of Oxaliplatin Combined with S-1 Capsule in the Treatment of Primary Carcinoma of Liver

目的对奥沙利铂联合替吉奥胶囊方案针对晚期肝癌临床治疗效果及毒副作用的探讨。方法我院2014年3月至2016年3月收治的46例晚期肝癌患者,均经细胞学和影像学证实,随机分为观察组23例:奥沙利铂+替吉奥胶囊方案;对照组23例:FOLF-OX4方案(奥沙利铂+亚叶酸钙+5-氟尿嘧啶)。结果对照组ORR为8.7%,DCR为55.5%;TTP为2.3月,MST为8.8月,半年PFS率为34.7%;观察组ORR为13%,DCR为56.5%;TTP为3.1月,MST为9.7月,半年PFS率为39.1%;观察组疗效稍高于对照组,但两组疗效差异无统计学意义(P>0.05);对照组消化道反应、血液学毒性反应均高于观察组,二者差异有统计学意义(P<0.05);两组神经毒性、肝肾功能毒性差异无统计学意义(P>0.05)。结论奥沙利铂为基础的联合化疗方案对晚期肝癌显示较好抗肿瘤效果,而口服给药的替吉奥胶囊对于不能耐受长期静脉化疗的病人可以选择。

Objectives To observe the clinical efficacy and toxic side effects of treating advanced hepatic cancer by oxaliplatin combined with S-1 capsules（Tegafur, Gimeracil and Oteracil Potassium Capsules）.Methods 46 patients with advanced lived cancer hospitalized from March, 2014 to March, 2016, who were all diagnosed by cy-tology and iconography, were divided into 23 groups randomly： Observing group： 23 patients underwent the regimen of oxaliplatin combined with S-1 capsules; referring group： 23 patients underwent FOLFLOX4 plan （oxali-platin ＋ calcium folinateH-5-fluorouracil） . Results Referring groups ： ORR 8.7%, DCR 55.5, TTP 2.3months; MST 8.8Months, PFS of half year 34.7%;Observing groups： OP^R 13%, DCR 56.5, TTP 3.1months; MST 9.7Months, PFS of half year 39.1%; figures of observing groups are higher than referring groups but the differenc-es of effects have no statistical significances（P〉 0.05） , the digestive tract effects and bloody effects of referring groups are both higher than observing groups. The differences between them have statistical significances（P〈 0.05）; the differences of damages to liver and kidney have no statistical significances （P〉0.05）.Conclusions Oxali-platin shows good effects for treating advanced liver cancer against tumors based on basic associated chemothera-py plan . However, the oral Tijiao could be used fro those patients who could not stand long - term vein che-motherapy.
