前瞻性护理干预在肝硬化并发肝性脑病病人的应用价值

Application Value of Proactive Nursing Intervention in Patients with Hepatic Encephalopathy Complicating Uncompensated Liver Cirrhosis

目的探讨前瞻性护理干预在预防失代偿期肝硬化患者并发肝性脑中的应用价值。方法将2014年1月~2015年12月116例失代偿期肝硬化并发肝性脑病患者随机分为实验组(58例)、对照组(58例),对照组患者住院治疗期间接受临床常规护理干预,实验组患者则接受前瞻性护理干预,比较两组干预后肝性脑病的意识障碍加重、并发症、再发率及患者对护理工作的满意度。结果实验组患者在意识障碍加重情况、并发症的发生及住院期间肝性脑病的再发生率均低于对照组,差异有统计学意义(P<0.05);实验组患者对护理工作的满意度高于对照组,P<0.05,差异具有统计学意义。结论实施前瞻性护理干预后可促进失代偿期肝硬化肝性脑病患者病情好转,减少并发症及降低复发率,提高护理满意度。

Objective To study the proactive nursing intervention in prevention of hepatic encephalopathy complicating uncompensated liver in patients with application value. Methods Between January 2014 and December2015,in our hospital 116 cases of patients with hepatic encephalopathy complicating uncompensated liver cirrhosis,as the research object. By using stochastic indicator were divided into the observation group and control group,58 cases of patients in each group. The control group patients accepted routine clinical nursing intervention during hospitalization,observation group patients received proactive nursing intervention. The difference in the rate of disturbance of consciousness, complication,recurrence and nursing satisfaction were compared between the two groups with proactive nursing and routine nursing during hospitalization.Results The disturbance of consciousness of the observation group and the control group were 13.7% and 18.9%, respectively. The difference was statistically significant(x^2=3.12,P<0.05). The complication、recurrence rate of the patients with observation group was significantly lower than that of the control group, and nursing satisfaction of the quality group was significantly higher than the control group(P<0.05). Conclusion Carrying out of proactive nursing care in patients with hepatic encephalopathy complicating uncompensated liver cirrhosis can improve the clinical treatment effect, prevent complication,recurrence of hepatic encephalopathy and increase nursing satisfaction.
