氧化亚氮/氧气吸入镇静术在老年人牙科焦虑症牙拔除术中的临床研究

Clinic study of nitrousoxide/oxygen inhalation sedation in the tooth of extraction of the elderly patients associated dental anxiety

目的观察氧化亚氮/氧气吸入镇静术（nitrousoxide/oxygen inhalation sedation,HIS）在老年人牙科畏惧症（dental anxiety,DA）患者牙拔除术中的疗效。方法选取2014年1月~2016年5月我院收治的两侧患牙情况相似的患者100例作为研究对象,随机选择一侧患牙为试验组,另一侧为对照组。试验组行氧化亚氮/氧气吸入镇静术联合盐酸阿替卡因麻醉;另一侧为对照组行盐酸阿替卡因麻醉。通过心电监护仪检测所有患者麻药注射时和拔牙过程中的血压、心率、血氧饱和度,疼痛感采用视觉评价量表（verbal analougu scale,VAS）测试,拔牙后填写Houpt行为量表。结果麻醉时和拔牙时,试验组收缩压、舒张压、心率和VAS值明显低于于对照组,差异有统计学意义（P〈0.05）。两组间Houpt行为评分比较,差异有统计学意义（P〈0.05）。结论氧化亚氮/氧气吸入镇静术对老年患者具有良好的止痛和镇静作用,能明显提高患者的依从性。

Objective To observe the clinical efects of nitrousoxide / oxygen inhalation sedation in the tooth of extraction of the elderly patients associated dental anxiety.methods From January 2014 to May 2016 in our hospital were similar on both sides of the teeth in 100 patients as the research object, randomly selected one side teeth as the experimental group, the other side as control group. The experimental group received the Nitrous Oxide / oxygen inhalation sedation of articaine hydrochloride combined anesthesia; the other side as control group for articaine hydrochloride anaesthesia. The ECG detection of all patients with anesthetic injection and extraction process of blood pressure, heart rate, oxygen saturation, pain by visual evaluation scale （verbal analougu scale, VAS） after extraction test, fill out the Houpt behavior Table.Results During anesthesia and tooth extraction, systolic blood pressure, diastolic blood pressure, heart rate and VAS value in the experimental group were signifcantly lower than those in the control group, the diference was statistically signifcant （P〈0.05）. There was signifcant diference between the two groups in the Houpt behavior score （P〈0.05）.Conclusion Nitrousoxide / oxygen inhalation sedation is efective in control of anxiety and pain and it can improve the compliance of elderly patients.
