植骨术联合烤瓷夹板修复对牙周骨缺损基牙龈沟液IL-1β水平和牙周修复的影响

Effect of bone grafting combined with porcelain plate repair on IL-1β levels and periodontal repair in gingival crevicular fluid of periodontal bone defects

目的探讨植骨术联合烤瓷夹板修复对牙周骨缺损基牙龈沟液白细胞介素(IL)-1β水平和牙周修复的影响。方法 84例牙周骨缺损牙周病患者,按修复方式将其分为观察组(植骨术联合烤瓷夹板修复)与对照组(植骨术),每组42例。对比两组患者IL-1β、附着丧失(CAL)情况及探诊深度(PD)的变化。结果治疗14、25周时,两组IL-1β水平较治疗1周时均下降;治疗14、25周时,观察组IL-1β水平分别为(73.55±9.73)、(63.39±8.09)ng/L,明显低于对照组的(170.22±47.57)、(117.24±39.72)ng/L,差异有统计学意义(P<0.05)。治疗14、25周时,两组患者CAL、PD水平均较治疗1周时明显下降;观察组患者CAL、PD在治疗1、14、25周时均低于对照组,差异有统计学意义(P<0.05)。结论植骨术联合烤瓷夹板修复较单一植骨术对白介素细胞IL-1β的改善作用更为明显,能有效促进牙周骨缺损基牙牙周组织的修复重建,疗效更佳,值得临床推广。

Objective To discuss the effect of bone grafting combined with porcelain plate repair on interleukin（IL）-1β levels and periodontal repair in gingival crevicular fluid of periodontal bone defects. Methods A total of 84 periodontal disease patients with periodontal defects receiving repair of periodontal defect were divided by repair methods into observation group（bone grafting combined with porcelain plate repair） and control group（bone grafting）, with 42 cases in each group. Comparison were made on IL-1β, clinical attachment loss（CAL） and probing depth（PD） changes in two groups. Results After 14 and 25 weeks of treatment, both groups had lower IL-1β level than 1 week of treatment. After 14 and 25 weeks of treatment, the observation group had obviously lower IL-1β level respectively as（73.55±9.73） and（63.39±8.09） ng/L than（170.22±47.57） and（117.24±39.72） ng/L in the control group, and their difference was statistically significant（P〈0.05）. After 14 and 25 weeks of treatment, both groups had obviously lower CAL and PD level than 1 week of treatment. The observation group had lower CAL and PD at 1, 14 and 25 weeks of treatment than the control group, and the difference was statistically significant（P〈0.05）. Conclusion Compared with single bone grafting, combination of bone grafting and porcelain plate repair is more effective for the improvement of interleukin IL-1β, and can effectively promote the repair of periodontal bone defects of periodontal tissue. It is worthy of clinical promotion.
