独活寄生汤治疗肝肾亏虚腰椎间盘突出症随机平行对照研究

Duhuojisheng Decoction in the Treatment of Liver and Kidney Deficiency of Lumbar Disc Herniation Randomized Controlled Study

[目的]观察独活寄生汤治疗肝肾亏虚腰椎间盘突出症疗效。[方法]使用随机平行对照方法,将38例住院患者按病志号抽签方法简单随机分为两组。对照组19例塞来昔布200mg/次,1次/d,口服。治疗组19例独活寄生汤(独活30g,熟地、杜仲各24g,白芍18g,牛膝、人参、威灵仙、当归、雷公藤、川芎各15g,桑寄生12g,秦艽、茯苓各10g,防风9g,甘草6g,细辛、肉桂、白花蛇舌草、全蝎各3g),疼痛加重添加川乌制、草乌制各9g;血瘀明显加甲珠、刘寄奴、泽兰各9g;寒湿明显加附片制9g;湿热明显加苍术9g,薏苡仁、黄柏各30g;下肢麻木加伸筋草20g,川断24g,木爪20g,药材冷水浸泡20min,水煎30min,煎汁300m L,100m L/次,3次/d,口服。治疗10d为1疗程。观测临床症状、疼痛评分、生活质量评分、不良反应。连续治疗2疗程(20d),判定疗效。[结果]治疗组显效11例,有效7例,无效1例,总有效率94.74%;对照组显效5例,有效9例,无效5例,总有效率73.68%;临床疗效两组无显著差异(P>0.05)。疼痛评分、生活质量评分两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01)。[结论]独活寄生汤加味治疗肝肾亏虚腰椎间盘突出症,疗效满意,无严重不良反应,值得推广。

[Objective] To observe Duhuo Jisheng decoction for treating deficiency of liver and kidney and lumbar intervertebral disc herniation. [Methods] Using random parallel control method, 38 patients were randomly divided into two groups according to the sick number drawing method. The control group, 19 cases, celecoxib 200 rag/times, 1 times/d, oral. The treatment group of 19 cases of Duhuo Jisheng decoction, increased pain add the 9g, Radix Aconiti kusnezoffii; blood stasis obviously and a pearl, Liujinu, the 9g was added adenophorum 9g; cold tablets; damp heat was 9g plus Cangshu, coix seed, Phellodendron 30g; lower extremity numbness and Lycopodii 20g, root 24g, wood claw 20g, cold water soaking herbs 20min Decoction 30min, Jianzhi 300mL, 100 mL/times, 3 times/d, oral. The course of treatment for 10d is 1. The clinical symptoms, pain score, quality of life score and adverse reactions were observed. Continuous treatment for 2 courses （20d）, to determine the efficacy. [Results] 11 cases were effective in the treatment group, 7 cases were effective, 1 cases were ineffective, the total effective rate was 94.74%; the control group was markedly effective in 5 cases, effective in 9 cases, ineffective in 5 cases, the total effective rate was 73.68%; the clinical curative effect of the two groups had no significant difference （P〉0.05）. The pain score and quality of life score were improved in the two groups （P〈0.01）, and the treatment group was better than that of the control group （P〈0.01）. [Conclusion] Duhuo Jisheng decoction in the treatment of liver and kidney deficiency of lumbar disc herniation, satisfactory curative effect, no serious adverse reactions, worthy of promotion.
