补中益气汤联合神经肌肉刺激治疗仪治疗产后盆底功能障碍性疾病随机平行对照研究

Randomized Parallel Controlled Study on the Treatment of Postpartum Pelvic Floor Dysfunction with Buzhong Yiqi Decoction combined with Neuromuscular Stimulator

[目的]观察补中益气汤联合神经肌肉刺激治疗仪治疗产后盆底功能障碍性疾病疗效。[方法]使用随机平行对照方法,将100例住院患者按病志号抽签方法简单随机分为两组。对照组50例神经肌肉刺激治疗仪,治疗头置于阴道内,电刺激配合生物反馈治疗,遵循个体化原则,针对患者自身情况及康复过程中效果及时调整,制定个体化模式和方案(频率和脉宽),20min/次,2次/周。治疗组50例补中益气汤:党参、枳壳、当归各10g,黄芪、益母草各15g,陈皮、升麻、柴胡各6g,白术9g,炙甘草6g,水煎150m L,1剂/d,早晚分服,连续服用10d;腹中痛,加白芍;头痛,加蔓荆子、川芎;咳嗽,加五味子、麦冬;气滞,加木香;神经肌肉刺激治疗仪治疗同对照组。连续治疗10d为1疗程。观测临床症状、盆底综合肌力恢复情况、盆底肌功能障碍疾病发生率、生活质量、不良反应。治疗1疗程(10d),判定疗效。[结果]肌纤维肌力治疗组优于对照组(P<0.01)盆底肌功能障碍疾病发生率治疗组低于对照组(P<0.01)。日常生活能力、社交能力、抑郁两组均有改善(P<0.01),治疗组改善优于对照组(P<0.01);焦虑治疗组明显改善(P<0.01),对照组无明显变化(P>0.05)。[结论]补中益气汤联合神经肌肉刺激治疗仪治疗产后盆底功能障碍性疾病,疗效满意,无严重不良反应,值得推广。

[Objective] To observe the curative effect of the treatment of postpartum pelvic floor dysfunction by Buzhong Yiqi decoction combined with neuromuscular stimulator. [Method] 100 patients were randomly divided into two groups according to the random number method. The control group of 50 patients with neuromuscular stimulation, the treatment head placed in the vagina, given electrical stimulation combined with biofeedback therapy, follow the principle of individuation, the effect of the patient＇s own situation and make timely adjustment of the rehabilitation process, individual mode and scheme, 20 rain/time, 2 times a week. The treatment group of 50 cases of Buzhong Yiqi： Dangshen, Zhiqiao, Danggui each 10g, Huangqi,Yimucao each 15g, Chenpi, Shengma, Chaihu each 6g, Baizhu 9g, Zhigancao 6g, decoction 150mL, 1 agent/d, 2 times/d, continuous use of 10d. Stomach pain, ad Baishao, headache, ad Manjingzi, Chuanxiong, cough, plus Wuweizi, Maidong; Qi stagnation,ad Muxiang; neuromuscular stimulation treatment with the control group. Continuous treatment of 10d for 1 courses. The clinical symptoms, pelvic floor muscle recovery, the incidence of pelvic floor dysfunction, quality of life and adverse reactions were observed. 1 treatment courses （10d）, to determine the efficacy. [Result] the treatment group was lower than that of the control group （P〈0.01） in the treatment group of muscle fiber strength than that of the control group （P〈0.01）. The ability of daily living, social ability, depression and anxiety were improved in the two groups, the treatment group was better than that of the control group （P〈0.01）. There was no significant change in the control group （P〉0.05）. [Objective] to study the effect of the treatment of postpartum pelvic floor dysfunction disease with Yiqi Yiqi decoction combined with neuromuscular stimulator.
