仪器辅助复位结合心理辅导治疗良性阵发性位置性眩晕

Treatment of benign paroxysmal positional vertigo by repositioning maneuver plus psychological intervention

目的观察耳石复位结合心理辅导治疗良性阵发性位置性眩晕的临床疗效。方法符合纳入标准的患者210例,随机分为手法复位治疗组、仪器辅助复位治疗组、仪器辅助复位结合心理辅导治疗组,每组70例。所有患者在其治疗前后均接受DHI评估。三组患者分别在治疗前、治疗后1周和1个月,评估其治疗效果。结果治疗后1周,三组间疗效存在有明显的差异(χ~2=10.287,P=0.006);治疗后1个月,三组间疗效亦存在有明显的差异(χ~2=10.183,P=0.006),其中手法复位治疗组和仪器辅助复位治疗组的疗效无明显差异,仪器辅助复位结合心理辅导治疗组疗效优于其他两组。三组患者治疗后1周和1个月时的DHI评分均有改善,仪器辅助复位结合心理辅导治疗组亦优于其他两组(P<0.05)。结论仪器辅助复位结合心理辅导治疗良性阵发性位置性眩晕具有较好的疗效。

Objective To evaluate the efficacy of device-assisted repositioning maneuver plus psychological intervention for treatment of benign paroxysmal positional vertigo.Methods A total of 210 patients with benign paroxysmal positional vertigo were enrolled in this study and they were divided randomly into three groups,70 patients in each group.Patients were treated with manual repositioning maneuver in group A,device-assisted repositioning maneuver in group B and device-assisted repositioning maneuver plus psychological intervention in group C.The efficacies of treatments used in three groups were evaluated one week and one month after treatments,and also all patients were assessed with DHI before and one week and one month after treatments.Results There were significant differences in the efficacy between three groups either one week（2=10.287,P=0.006） or one month（2=10.183,P=0.006） after treatments.The efficacy in group C was better than that in group A or group B,but with no significant difference in the efficacy between group A and group B.DHIs of patients in three groups were improved one week and one month after treatments compared to those before their treatments,and with better improvement in patients of group C compared to other two groups（P〈0.05）.Conclusion The device-assisted repositioning maneuver plus psychological intervention is an effective treatment for benign paroxysmal positional vertigo,with a good efficacy.
