炎性标记物预测首发精神分裂症治疗应答的临床研究

INFLAMMATORY BIOMARKERS FOR PREDICTION OF TREATMENT RESPONSE IN FIRST-EPISODE SCHIZOPHRENIA

目的探讨炎性标记物与首发精神分裂症治疗应答之间的关系。方法首发精神分裂症病人64例,应用抗精神病药物治疗12周,缓解30例,未缓解34例。治疗前检测病人血清白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)和干扰素-γ(IFN-γ)水平。并以59例健康体检者作为正常对照组。结果病人组IL-4、IL-6、IL-8、IL-10、TNF-α和IFN-γ水平显著高于正常对照组(t=13.50-25.60,P<0.01);未缓解组IL-4、IL-6、IL-8、IL-10、TNF-α和IFN-γ水平显著高于正常对照组(t=3.33-20.71,P<0.01),缓解组IL-6、IL-8、TNF-α和IFN-γ水平显著高于正常对照组(t=10.46-20.71,P<0.01);未缓解组IL-6和IFN-γ水平显著高于缓解组(t=19.05、12.13,P<0.01)。结论首发精神分裂症病人炎性细胞因子水平显著增高,炎性细胞因子可作为判断预后的生物指标。

Objective To investigate the ability of inflammatory biomarkers to predict treatment response in first-episode schizophrenia. Methods Sixty-four patients with first-episode schizophrenia were selected to receive antipsychotic drugs for 12 weeks;30 patients achieved remission,and 34 patients did not achieve remission.The levels of interleukin（IL）-1β,IL-2,IL-4,IL-6,IL-8,IL-10,tumor necrosis factor-α（TNF-α）,and interferon-γ（IFN-γ）were measured before treatment.Fifty-nine matched healthy subjects were used as normal controls. Results Before treatment,the patient group showed significantly higher levels of IL-4,IL-6,IL-8,IL-10,TNF-α,and IFN-γthan the normal control group（t=13.50-25.60,P0.01）.After 12 weeks of treatment,when compared with the normal control group,the non-responders showed significantly higher levels of IL-4,IL-6,IL-8,IL-10,TNF-α,and IFN-γ（t=3.33-20.71,P0.01）;responders showed significantly higher levels of IL-6,IL-8,TNF-α,and IFN-γ（t=10.46-20.71,P0.01）.The non-responders had significantly higher IL-6 and IFN-γlevels than the responders（t=19.05 and 12.13,P0.01）. Conclusion Patients with first-episode schizophrenia show raised levels of inflammatory cytokines,and these inflammatory cytokines can be used as possible predictors of treatment response.
