血清肿瘤标记物在早期小细胞肺癌诊断中的临床价值

CLINICAL VALUE OF SERUM TUMOR MARKERS IN DIAGNOSIS OF EARLY-STAGE SMALL CELL LUNG CANCER

目的观察血清肿瘤标记物在早期小细胞肺癌诊断中的临床价值。方法随机选择2014年1月—2015年12月于我院进行早期小细胞肺癌治疗的病人60例作为观察组。随机选择同期住院的肺部实性病灶病人60例作为对照组,对所有病人进行肿瘤标记物检测,比较肿瘤标记物三联、四联检测的阳性率。结果观察组病人血清癌胚抗原(CEA)、细胞角蛋白21-1(CYFRA21-1)、神经元特异性烯醇化酶(NSE)、糖类抗原125(CA125)水平明显高于对照组(t=19.985~46.924,P<0.01)。对小细胞肺癌病人肿瘤组织中CEA、CYFRA21-1、NSE、CA125等肿瘤标记物的三联、四联检测结果显示,四联检测的阳性率较高,达到85.71%。结论血清肿瘤标记物在早期小细胞肺癌诊断中具有重要价值,尤其是多项肿瘤标记物联合检测,能够明显提高小细胞肺癌的早期检出率。

Objective To investigate the clinical value of serum tumor markers in the diagnosis of early-stage small cell lung cancer. Methods A total of 60 patients with early-stage small cell lung cancer who were treated in our hospital from January 2014 to December 2015 were enrolled as observation group,and another 60 patients with solid lung lesions who were hospitalized during the same period of time were enrolled as control group.Tumor markers were measured for all patients,and the positive rates of triple and quadruple measurement of tumor markers were compared. Results The observation group had significantly higher serum levels of carcinoembryonic antigen（CEA）,cytokeratin 21-1（CYFRA21-1）,neuron-specific enolase（NSE）,and carbohydrate antigen 125（CA125）than the control group（t=19.985-46.924,P0.01）.The triple and quadruple measurement of tumor markers such as CEA,CYFRA21-1,NSE,and CA125 in small cell lung cancer patients showed that the positive rate of quadruple measurement reached 85.71%. Conclusion Serum tumor markers play an important role in the diagnosis of earlystage small cell lung cancer,and the combined measurement of various tumor markers can significantly improve the early detection rate of small cell lung cancer.
