Orthofix肢体延长技术在儿童外伤性桡骨短缩畸形治疗中的应用

BONE LENGTHENING WITH ORTHOFIX EXTERNAL FIXATOR IN TREATMENT OF TRAUMATIC RADIAL SHORTENING DEFORMITY IN CHILDREN

目的探讨Orthofix肢体延长技术治疗儿童外伤性桡骨短缩畸形的效果。方法回顾性分析2012年6月—2015年8月我院收治的6例外伤性桡骨短缩畸形病儿的临床资料。6例病儿均有桡骨远端骨骺骨折史,X线摄片证实桡骨远端发生骨骺早闭,应用Orthofix单臂外固定架固定并采用短缩的桡骨截骨延长技术进行治疗。根据外观和功能评价临床疗效。结果 6例桡骨短缩畸形均得到矫正。术后随访17~36个月,截骨部位愈合良好,腕关节屈曲、背伸活动良好,桡偏畸形得到纠正,前臂旋转功能良好,无肢端循环不良和骨不连。发生针道感染1例。结论 Orthofix肢体延长技术可以纠正骨骺损伤导致的桡骨短缩畸形,明显改善关节活动功能,创伤小,并发症少,操作方便,是一种较为可靠有效的方法。

Objective To analyze the clinical effect of bone lengthening with Orthofix external fixator in children suffering from traumatic radial shortening deformity. Methods The clinical data of 6 patients with traumatic radial shortening deformity,who were admitted to our hospital from June 2012 to August 2015 and who underwent bone lengthening with single-arm Orthofix external fixator,were retrospectively analyzed.The 6 patients had a history of distal radialus epiphysealmetaphyseal fracture,and premature closure of distal radial epiphysis confirmed by X-ray examination.Clinical effects were assessed according to the function and appearance of the radius. Results All patients were followed up for 17 to 36 months.The deformity of all patients was improved significantly after surgery.The osteotomy was healing well,with good wrist flexion/extension,and radial deviation was corrected,with good forearm pronation/supination.There were no poor acral circulation and nonunion.One patient developed pin infection. Conclusion Bone lengthening with single-arm Orthofix external fixator can correct radial shortening deformity caused by epiphyseal injury,which is associated with a good range of motion of joint,low incidence of complications,and little operative trauma.It is a reliable and effective method for radial shortening deformity caused by epiphyseal injury in children.
