降脂颗粒剂治疗29例HAART后高脂血症患者的疗效观察

Observation on therapeutic efficacy of Jiangzhi granules for treatment of HAART-related hyperlipidemia in 29 patients

目的探讨降脂颗粒剂治疗高效抗反转录病毒治疗(HAART)后高脂血症患者的临床疗效。方法采用实用性随机对照研究的实验方法,将58例HAART后高脂血症的受试者随机分为实验组(HAART+中药降脂颗粒)、对照组(HAART+安慰剂),每组29例,同时测定治疗前、治疗后12、24周血脂、肝肾功能等指标。两组均治疗6个月。结果在实验组中,降脂颗粒剂在治疗24周时可以显著降低甘油三酯(TG)的水平,与治疗前比较差异有统计学意义(t=2.601,P=0.013);在治疗12、24周,可提高高密度脂蛋白(HDL-C)的水平,与治疗前比较差异有统计学意义(12周t=-2.562,P=0.016;24周t=-2.450,P=0.021);而对照组总胆固醇、TG、HDL-C、低密度脂蛋白治疗后均无统计学意义。治疗12、24周时,实验组HDL-C的治疗效果比对照组好(12周t=-2.573,P=0.013;24周t=-3.041,P=0.004);治疗24周时,实验组TG的治疗效果比对照组好(t=2.601 P=0.013)。治疗24周时,实验组动脉粥样硬化指数下降与对照组相比差异具有统计学意义(t=-2.707,P=0.01)。结论该降脂颗粒剂治疗HAART后高脂血症可以改善脂代谢异常,减少HAART的不良反应,降低心血管疾病的风险。

Objective To evaluate the clinical curative effect of hyperlipidemia with the treatment of Jiangzhi granules for the AIDS patients receiving highly active antiretroviral therapy（HAART）. Methods 58 patients with hyperlipidemia after HAART were randomly divided into experimental group （HAART＋ Chinese medicine Jiangzhi granules）, and control group （HAART＋ placebo） with 29 cases in each. Both groups were treated for 6 months, during which blood lipid, and liver/kidney function indexes were tested before and after treatment for 12 weeks, and 24 weeks. Results In the experimental group the treatment based on J iangzhi granules could significantly reduce the level of TG （t= 2.061, P= 0. 013） and also increased the level of LDL-C （12weeks t= -2. 562, P：0. 016; 24weeks t= -2. 450, P=0. 021）. However, TC, TG, HDL-C, LDL-C in the control group after treatment had no statistically significant changes. The HDL-C in the experimental group was better than that in the control group after treatment for ]2 weeks, and 24 weeks（12weeks t= 2. 573, P=0. 013; 24weeks t=-3. 041, P=0. 004）. The TG in the experimental group was better than that in the control group （t=2. 601 P=0. 013） after 24 weeks of treatment. After 24 weeks of treatment, atherosclerosis index of the ex- perimental group decreased significantly compared with that of the control group （t= -2. 707 P= 0. 01 ）. Conclusion Jiangzhi granule treatment of hyperlipidernia after HAART can regulate lipid metabolism disorder, and reduce the side effects of HAART and the risk of cardiovascular disease.
