西酞普兰联合中药治疗中风后抑郁症的临床观察

Efficacy Observation of Citalopram combined with Traditional Chinese Drug in the Treatment of Post-stroke depression

目的观察中药联合西酞普兰治疗中风后抑郁症患者临床疗效。方法将80例中风后抑郁(PSD)患者随机分为两组,对照组:口服西酞普兰片,20 mg/次,1次/d,早餐后口服;治疗组:在对照组基础上加中药;最终完成临床观察75例(对照组38例,治疗组37例)。干预6周后,比较分析两组患者的汉密顿尔抑郁量表(HAMD)评分、阿森斯失眠量表(AIS)评分、主要临床症状、康复训练依从性、临床治疗有效率及不良反应发生率。结果治疗后,治疗组患者HAMD、AIS评分和主要临床症状评分均明显降低,且低于对照组,差异有统计学意义(P<0.05);康复训练依从性与对照组比较,差异无统计学意义(P>0.05);治疗组不良反应发生率明显低于对照组,且临床治疗有效率明显高于对照组(P<0.05)。结论在治疗PSD上,西酞普兰联合中药能有效改善主要临床症状,提高临床治疗效果,同时也减少了不良反应发生率,经验值得推广。

Objective To compare the clinical effects of citalopram combined with traditional Chinese drug used for patients with Post-stroke depression（PSD）. Methods 80 Post-stroke depression patients were selected and randomly divided into treatment group and control group with 40 cases in each group. Control group was given citalopram, 20 mg, qd, after breakfast;Treatment group was given traditional Chinese drug and citalopram as the same as control group. As a results, there amounted to 75 patients who completed the clinical observation, including 38 cases in the control group and 37 cases in the treatment group. After 6 weeks treatment, HAMD score,AIS score, main clinical symptoms,rehabilitation compliance, clinical treatment and the incidence of adverse reactions were analyzed.Results After treatment,HAMD and AIS score of treatment group were decreased obviously,and they were lower than those in control group,with statistical significance（P〈0.05）. There was no significant difference in the rehabilitation compliance between the treatment group and the control group（P〈0.05）. The incidence of adverse reactions in the treatment group was significantly lower than that of the control group, and the clinical treatment efficient in the treatment group was significantly higher than that in the control group（P〈0.05）.Conclusion Citalopram combined with traditional Chinese medicine can effectively improve main clinical symptoms and the effect of clinical treatment, but also it can reduce the incidence of adverse reactions. It is worthy of clinical promotion.
