关节镜联合胫骨高位截骨术治疗内翻型膝关节骨性关节炎的临床效果研究

Study on the the clinical effect of arthroscopy combined with high tibial osteotomy in the treatment of varus knee osteoarthritis

目的探讨内翻型膝关节骨性关节炎予以关节镜及胫骨高位截骨术的临床意义。方法研究对象选取2011年4月12日~2012年6月29日我院58例内翻型膝关节骨性关节炎患者,以平均、随机为原则进行分组。对照组采取全膝关节置换治疗,观察组采取关节镜及胫骨高位截骨术。观察两组膝关节功能、疼痛、生活质量及并发症发生情况。结果治疗后,观察组膝关节功能评分、疼痛评分、生活质量评分分别为(81.85±1.80)分、(2.58±0.49)分、(85.33±3.21)分,均优于对照组,差异有统计学意义(P<0.01);观察组并发症发生率(3.45%)低于对照组的20.69%,差异有统计学意义(P<0.05)。结论内翻型膝关节骨性关节炎采取关节镜及胫骨高位截骨术治疗,效果确切、安全可靠。

Objective To investigate the clinical significance of arthroscopy combined with high tibial osteotomy in the treatment of varus knee osteoarthritis. Methods 58 patients with varus knee osteoarthritis who were admitted to our hospital from April 12, 2011 to June 29, 2012 were selected as the study subjects. The patients were grouped according to the average and random principle. The patients receiving total knee replacement were selected as the control group, and the patients receiving arthroscopy combined with high tibial osteotomy were selected as the observation group. The knee function, pain, quality of life and complications were observed in both groups. Results After treatment, the scores of knee function, pain score and score of quality of life were（81.85±1.80）,（2.58±0.49） and（85.33±3.21） respectively, which were all better than those in the control group, and the differences were statistically significant（P0.01）; the incidence rate of complications in the observation group（3.45%） was lower than that in the control group（20.69%）, and the difference was statistically significant（P0.05）. Conclusion Arthroscopy combined with high tibial osteotomy in the treatment of varus knee osteoarthritis is effective, safe and reliable.
