摘要
目的探讨C反应蛋白、白细胞介素和心肌酶谱与儿童支气管肺炎的关系及临床意义。方法选择2015年1月—2017年6月来我院就诊的1~5岁支气管肺炎患儿共580例为研究对象,组成支气管肺炎组,组内患儿根据病情的严重程度分为轻型肺炎组(n=466)和重型肺炎组(n=114)。并随机入选同时期来我院体检门诊查体的儿童580例组成正常儿童组作为对照。每位儿童均测定C反应蛋白、白细胞介素-6和心肌酶谱,比较各组之间变量是否存在差异。结果支气管肺炎组C反应蛋白、白细胞介素-6、乳酸脱氢酶、肌酸激酶、肌酸激酶同工酶分别为(13.6±3.9)ng/L(、37.4±5.2)ng/L(、302.5±27.4)U/L、(300.2±10.6)U/L、(30.9±6.1)U/L;正常儿童组分别为(5.9±1.4)ng/L、(12.1±0.9)ng/L、(139.3±20.1)U/L、(98.9±18.7)U/L、(14.1±3.4)U/L,两组间各指标差异均具有统计学意义(P<0.05)。同时发现,轻型肺炎组C反应蛋白、白细胞介素-6、乳酸脱氢酶、肌酸激酶、肌酸激酶同工酶分别为(11.4±3.5)ng/L、(35.4±7.2)ng/L、(278.4±26.3)U/L、(285.2±8.7)U/L、(26.9±6.1)U/L;重性肺炎组分别为(20.6±5.5)ng/L、(44.7±1.6)ng/L、(383.3±29.1)U/L、(360.9±12.1)U/L、(38.1±7.0)U/L,两组间各指标差异均具有统计学意义(P<0.05)。结论 C反应蛋白、白细胞介素、心肌酶谱的改变有助于支气管肺炎的诊断,对于判断支气管肺炎的严重程度也有一定的价值。
Objective To study the clinical significance about C-reactiveprotein, interleukins and myocardial enzyme spectrum in childrenbronchial pneumonia. Methods 580 cases of children patients withbronchopneumonia from January 2015 to June 2017 in our hospital atthe period from 1 to 5 years old, were selected as the research object,namely bronchial pneumonia group, according to the severity of thedisease, this group was divided into light pneumonia group (n=466)and heavy pneumonia group (n=114). 580 cases of normal childrenwho came to the hospital for medical examination at the same timewere randomly selected as control group. Every child’s C-reactiveprotein, interleukin-6 and myocardial enzyme spectrum weremessured, the differences about these results between two groupswere compared. Results Bronchial pneumonia group’s C-reactiveprotein, interleukin-6, lactate dehydrogenase, creatine kinase, creatinekinase isoenzyme were (13.6±3.9) ng/L, (37.4±5.2) ng/L, (302.5±27.4)U/L, (300.2±10.6) U/L, (30.9±6.1) U/L, when normal childrenwere (5.9±1.4) ng/L, (12.1±0.9) ng/L, (139.3±20.1) U/L, (98.9±18.7)U/L and (14.1 ± 3.4) U/L. Each index difference between the twogroups had statistically significant (P 〈 0.05). It also found that thelight pneumonia group’s C-reactive protein, interleukin-6, lactatedehydrogenase, creatine kinase, creatine kinase isoenzyme were(11.4±3.5) ng/L, (35.4±7.2) ng/L, (278.4±26.3) U/L, (285.2±8.7) U/L,(26.9±6.1) U/L, while heavy pneumonia group were (20.6±5.5) ng/L,(44.7±1.6) ng/L, (383.3±29.1) U/L, (360.9±12.1) U/L and (38.1±7.0)U/L. Each index difference between the two groups had statisticallysignificant (P 〈 0.05). Conclusion C-reactive protein, interleukin, thechange of myocardial enzyme spectrum are helpful to the diagnosisof bronchial pneumonia, and also have a certain value in judging theseverity of bronchial pneumonia.
